Gentlemen:

This refers to the investigation conducted by Mr. R. K. Herr of our staff on December 1-4, 1981, at your facility in Woodward, Oklahoma concerning allegations dealing with improper handling of radioactive sources.

The investigation and our findings are discussed in the enclosed investigation report.

During this investigation, it was found that certain of your activities were not conducted in full compliance with NRC requirements.

Consequently, you are required to respond to this matter, in writing, in accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Your response should be based on the specifics contained in the Notice of Violation attached to this letter.

We also found that your practices for securing and storing of radioactive sources was not acceptable.

Since these practices did not violate a specific NRC requirement, no citation is issued in this area.

We are, however, most concerned about this issue, and you are requested to reevaluate the security of your radioactive sources.

The results of this review and any corrective action which you plan to take should be addressed in your response to the cited violations.

The response directed by this letter and the accompanying Notice is not subject to the clearance procedures of the Office of Management and Budget as requried by the Paperwork Reduction Act of 1980, PL 96-511.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure will be placed in the NRC Public Document Room unless you notify this office, by telephone, within 10 days of the date of this letter and submit 1
