Response to Inspection 50-309/81-30, RWP Violation
Dear Sir:
In reply to the violation identified in the Notice of Violation, Appendix A of Reference (b), the following information is submitted:
Findings:
A group of workers entered a high radiation area without first reading and signing their Radiation Work Permits (RWP) and without carrying a survey meter. This is contrary to Technical Specification 5.11 and Health Physics procedure 9.1.10.
Response: The workers were all contractor personnel and had all received the standard indoctrination training in Health Physics. However, the detailed procedure for completing RWPs and the Technical Specification governing High Radiation Area entries was not covered in the indoctrination training.
This deficiency has been brought to the attention of the Training Department for correction.
Indoctrination training lesson plans have been revised to include HRA entry requirements and to stress the need for all members of the working party to read and sign RWPs.
This matter was discussed with the contractors involved and will be discussed at a general plant training meeting before March 1,1982 In addition, this matter has been discussed witn the Health Physics technicians who have been reminded of the requirements and have been instructed to increase their surveillance of contract workers.
