Appendix NOTICE OF VIOLATION Ecological Analysts,.Inc.
License No. 12-20156-01 As a result of the inspection conducted on January 27, 1982, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
Condition No. 13 of your license requires that you wipe test the external surface of the final source container of incoming packages containing licensed material.
Contrary to the above requirement, it was determined through state--
ments by-licensee representatives that this condition is not being met.
Specifically, the required wipe tests have not been performed since the inception of this license, June 22, 1981.
This is a Severity Level V violation (Supplement VII).

10 CFR 20.203(e) requires that rooms in which specified amounts of licensed material are used or stored be conspicuously posted " Caution Radioactive Material."
Contrary to the above requirement, as of the day of the inspection you failed to post both the radioactive storage area and the laboratory area where the licensed material is used and stored.
This is a Severity Level VI violation (Supplement VII).

10 CFR 19.11(a) and (b) require that current copies of Part 19, Part 20, your license, license conditions, documents incorporated into the license, license amendments and operating procedures be posted, or
.that a notice describing.these documents and where they may be examined, be posted.
10 CFR 19.11(c) requires that Form NRC-3, " Notice to Employees" be posted.
Contrary to the above requiremant, as of the day of the inspection, neither the documents nor the notices were posted.
This is a Severity Level VI violation (Supplement VII).
