To be used for well logging.

l The source shicids (transportation packages), source holders and logging tools are manufactured to Schlumberger specifications.

The transportation packages meet the DOT specifications for a "7A" package.

All sources (within a model number) are identical in dimensions and output; therefore, the shields, source holders and logging tools are interchangeabic.

Schlumberger will not exceed 450 grams of Plutonium as PuBe sources

at any single site.

Users will be Schlumberger Field Engineers.

An outline of the quali-fications of the Schlumberger Field Engineer is shown in. Attachment number 3.

t The qualifications of the Radiation Safety Officer (C. E. Racster) are shown in Attachment number 4.

The main office of the licensee is 5000 Gulf Freeway,llouston, Texas, 77023.

It is requested that the license permit storage at this ad-dress and any field facility of the licensee.

It is requested that use be permitted at the main office, any field facilit,. and temporary job sites of the licensee.

License Renewal - SNM-287 Page 4 In Attachment number 5 we show the handling tool provided for these sources (in source holder) and the attachment on the source holder for the handling tool.

Storage in each facility are constructed to provide surface radiation not to exceed regulatory standards and a padlocking system to provide security from theft.

Each field facility and the main office have at least one Ludlum II (or equivalent) survey meter.

The Ludlum II uses a geiger detector and has three scales; 0-0.5, 0-5.0 and 0-50 mr/hr.

Calibration is done locally using a 100 microcurie Radium 226 source and multi -

position jig.

The jig provides two-point calibration on each scale.

Calibration is performed after significant repairs and at least each six-months.

Attached are two copics of Schlumberger Radiation Sources and Controls Manuals which cover our radiation safety procedures as required in Section "G" of the application guide.
