Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 File 0260/15525/15526 PRD-81/44, Interim Report No. 2, Unqualified Bettis Air Actuators on Henry Pratt Valves AECM-82/45 On October 29, 1981, Mississippi Power & Light Company notified Mr. P. A.

Taylor, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns unqualified Bettis Air Actuators on Henry Pratt Valves.

Our investigation into reportability, cause, and extent is continuing.

Details are included in our attached interim report. We expect to submit a final report by March 15, 1982.

  • Page 1 of 2 INTERIM REPORT NO. 2 FOR PRD-81/44 I. Description of the Deficienc.E Bettis Air Actuators funished with certain Henry Pratt butterfly valves under specifications M-257.0 and M-258.0 are not typical of the units utilized for original Qualification Testing.

Investigative data indicates that the delivered valve actuators do not match the units originally qualified. The difference between the delivered actuators and the test units consists of materials of construction in all cases and design in some cases. Hence, a direct correlation to verify functional operatility cannot be made at this time.

The defective cetuators are located in Unit I and Unit 2.

The deficiency is known to af fect the Containment Cooling System (M41),

the Auxiliary Building Ventilation System (T41), the Fuel Handling Area Ventilation System (T42) and the Control Room HVAC System (Z51).

Investigation is being undertaken to determine applicability to other systems and to the NSSS vendor.

The potential exists that the valves would not function under seismic and environmental conditions as required by our Constructor's specifi-cations. At this time the functional operability of the valves cannot be determined. If the condition had remained uncorrected, it is possible that the actuators may not perform their intended safety function.

We are continuing our investigation into reportability under the provi-sions of 10CFR50.55(e).

II. Approach to Resolution of the Problem i

I The specific cause of the deficiency is that the actuators were not manufactured under a QA program by the G. H. Bettis Company.

Investigations are underway to determine the specific number of valves affected and to determine acceptability of the actuators as to seismic qualifications, ef fect on safety, and reportability.

j III. Status of Proposed Resolution l

It has been determined that failure of the actuators due to degradation l of the internals would not affect safety. The actuators are spring l

return models and will fail in the safe position.

MP&L, however, has elected to upgrade the seals, lubricants, and coat-ing on the pins and rollers prior to commercial operation. This will minimize future plant outage time in the case of a harsh environment condition which could degrade these materials.

IV. Reason Why A Final. Report Will Be Delayejd

- Mississippi Power and Light is investigating the acceptability as to seismic qualifications of the actuators that were not manufactured under a- QA program.
