l APPENDIX A NOTICE OF VIOLATION Fort St. Vrain Nuclear Generating Station Docket: 50-267 Public Service Company of Colorado License:

DPR-34 Based on the results of an NRC inspection conducted during the period November 16-20, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

Failure to Perform Surveillance Testing at the Frequency Specified in the Technical Specifications Contrary to the requirements of the Technical Specifications, the licensee failed to perform surveillance testing at the specified -

frequency for three surveillance tests described below:

1.

Technical Specification 5.2.10(a)(3) requires that the diesel engine for the diesel driven fire pump be inspected each refueling shutdown.

Contrary to this requirement, the diesel engine for the diesel driven fire pump was not inspected during the May 1981 refueling shutdown.

It was last inspected in September 1980.

2.

Technical Specification 5.10.1 requires an annual actuation test of the heating and ventilation dampers and fans in the three room control complex.

Contrary to this requirement, this surveillance test was last performed on July 17, 1980, an interval of 16 months to the date of-this NRC inspection.

This interval is in excess of the

+25 percent adjustment to surveillance intervals permitted by the Technical Specification definition (2.18) of surveillance intervals.

Technical Specifications 5.10.2(a) and (b) require that the pressure and weight of the Halon storage cylinders be checked quarterly.

Contrary to this requirement, Halon storage cylinder weight and pressure checks were not performed between February 20, 1981, and July 1, 1981, an interval of 19 weeks.

This interval is in excess of the +25 percent adjustment to surveillance intervals permitted by the Technical Specification definition (2.18) of surveillance intervals.

These items constitute a Severity Level V violation (Supplement I.0).

Pursuant to the provisions of 10 CFR Part 2.201, Public Service Company of Colorado is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid violations; and (3) the date when full compliance will be achieved.

Under this authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Consideration may be given to extending your re-sponse time for good cause shown.
