Waterford SES Unit No. 3 USNRC Inspection Report 50-382/Rpt. 81-29
Dear Mr. Madsen:
The following information regarding the violation cited by the USNRC Inspectors in IE Inspection Report No. 50-382/Rpt. 81-29 dated December 11, 1981, is herewith submitted.
A.
Failure To Follow Procedures For The Protection Of Electrical Penetration Assemblies Criterion V of Appendix B to 19CFR50 requires that activities affecting quality be prescribed by documented instructians, procedure s, or drawings of a type appropriate to the circumstances, and that they shall be accomplished in accordance with these instructions, procedures, or l
drawings.
Ebasco CMI 28, Revision 8, " Care and Maintenance Instructions for Electrical Penetration Assemblies", require s that, during in place storage penetration assemblies be provided with adequate protection from damage and deterioration as a result of conditions or activities in the vicinity.
Contrary to the above, during a plant tour on November 2,1981, the NRC inspector discovered protective barriers absent from Class IE electrical penetrations 107SB and 10lSA. Penetrations in such an unprotected condition are subject to damage due to adjacent construction activities.
This is a Severity Level V violation (Supplement II.E).

Corrective Action Taken and the Results Achieved:
Immediate action was initiated by LP&L and the Contractor to assure 8202020158 820121

o...s Mr. G. L. Madsen Page 2' that the electrical penetrations were protected. Alliprotective' barriers are installed and. damaged penetrations documented.'
- 2) Corrective Action Taken to Preclude Repetition:
Contractor Quality Control has increased the surveillance of the Electrical Penetrations from a monthly to a weekly basis. The construction supervisor instructed the craf ts to replace the barriers af ter completing' their work.
The Contractor has assigned. a Quality Control Inspector to _ inspect nightly all Electrical Penetrations to verify that protective barriers have been installed.
- 3) Date When Full Corrective Action Will Be Achieved:
The Care and Maintenance of Electrical Penetrations is an on going activity
.until completion of the project.
Presently the Contractor is in.
compliance.
