NOTICE OF VIOLATION

As a result of the inspection conducted on December 16, 1981, and in ac-I cordance with the Interim Enforcement' Policy, 45 FR 66754 (October 7, 1980),

l the following violations were identified:

License. Condition 14 requires the licensee to possess and use licensed material'in accordanu with statements, representations,'and procedures 1

contained in application dated September 1, 1978.

I 1.

In the referenced application, the licensee indicates that the pro-cedures described in Appendix D Section 2 will be used for calibration of the dose calibrator. Appendix D Section 2 (of NUREG-0338, Revision 1, dated November 1, 1977) requires that the dose calibrator linearity be checked at installation and quarterly thereafter.

Contrary to this requirement, the licensee failed to check the dose calibrator linearity from August 30, 1979 to July 31, 1981 and no check after July 31, 1981, periods in excess of a calendar quarter.

This is a Severity Level V violation (Supplement VII).

. Appendix D Section 2 requires that the licensee measure and record the l

activity of at least one reference source on a daily basis or before each use of the dose calibrator.

i Contrary to this requirement, the licensee failed to record the measured i

activity of their reference source on November 13, 16 and 23, 1981 and i

j December 11, 1981, and other dates during which licensed material was administered to patients.

This is a Severity Level VI violation (Supplement VII).

l 3.

10 CFR 20.401(a) requires each licensee to maintain records showing the radiation exposures of all individuals for whom personnel monitoring is' required and such records be kept on Form NRC-5 or contain all the information required by that form.

Contrary to this requirement, the licensee's radiation exposure records

- do not include the date of birth and social security number for all l

fndividuals for whom personnel monitoring is required.

This is a Severity Level VI violation (Supplement IV).
