s APPENDIX A NOTICE OF VIOLATION Washington Hospital Center Docket No.: 70-01500 Washington, D.C.
20010 License No.: SNM-1446 As a result of the inspection conducted on September 1,1981, and in accorccnce with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Prior to June 9,1981 Condition 9.B of License No. SNM-1446 limited the implantation of nuclear powered pacemakers to Coratomic Model C101.
Contrary to the above, from February 22, 1980 to June 2, 1981 you implanted four Coratomic Model C-10lP pacemakers, a model not authorized by your license.
This is a Severity Level IV Violation (Supplement VII)
Pur uant to the provisions of 10 CFP 2.201, the Washington Hospital Center is hereby required to submit to this cffice within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have bean taken and the results achieud; (2) corrective steps which will be taken to avoid further violations; and (3) the date wNn full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Art of 1980, PL 96-511.
