The licensee shall obtain all information from the foreign exporter necessary e
to complete the shipper's portion of Form NRC-741, " Nuclear Material Trans-mN actions Report."
If the licensee is the r,eceiver of the material, he shall h,
record both the shipper's and receiver's information on the Form NRC-741 and i
transmit pursuant to Section 40.64 of 10 CFR Part 40.
If the licensee is not y!
the receiver of the material, he shall provide the shipper's information to g*
the receiver of the material for inclusion on the receiver's Form NRC-741.
4N 2.
The licensee shall notify the appropria^e regional Safeguards Branch, Office of Inspection and Enforcement, inaediately, by telephona or telegram, if entry of any import shipment authorized by this license is refused by the U.S. Customs Service.

This license authorizes import only and does not authorize the receipt, physical Q
possession, or use of the nuclear material.
