The final paragraph.of the request for additional information requested the inclusion of letters of agreement between the licensee and transporting and
]
medical facilities.
Letters have been requested from the Dayton Fire Department and Dayton Fire Department Paramedics (transporting organization) and from St. Elizabeth Hospital (medical facility).
Due to the shortness of the response time available to us, only one letter (from the Dayton Fire Department Paramedics) has been received and included in the revised material.
Copies of the remaining two letters will be sent to you promptly.after their receipt by us.
