This refers to the routine safety inspection conducted by Dr. W. J. Adam and Ms. P. Whiston of this office on January 6-8, 1982, of activities authorized by NRC Byproduct Material Licenses No. 12-00074-04, No.'12-00074-05, No. 12-00074-07, and No. 12-00074-08 and to the discussion of our findings with Dr. Pinsky, Mr. Francis, Dr. Byron and others of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews'with personnel.
This inspection also included a measurement of the output of your tele-therapy equipment. Our measurement was made using a'Victoreen Model 570 R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80 centimeters and the field size used was 10 by 10 centimeters. After applying standard correction factors for temperature,. pressure, and timer error, our measure-adent of the output of your teletherapy equipment was 247.3 roentgens per minute. The most recent monthly spot check measurement of your teletherapy equipment, conducted on January 4, 1982, and extrapolated for January 8, 1982 reported a' measured output for the same exposure parameters of 242.7 roentgens per minute.

Based on the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.
l In addition to the above areas, the inspectors examined actions described in your letter dated July 19, 1978, regarding apparent items of noncomplianc found during our May 18-23, 1978 inspection. We have no further questions j
regarding these matters.

JAN 211982 flichael Reese IIospital and tiedical Center During this inspection, certain of your activities conducted under License No. 12-00074-04 appeared to be in noncompliance with NRC requirements, as specified in the enclosed Appendix. A written response, submitted under oath or affirmation, is required.
No items of noncompliance with NRC requirements were identified during the course of this inspection for Licenses No. 12-00074-05, No. 12-00074-07, and No. 12-00074-08.
The responses directed by this letter (and the accompanying Notice) are not subject to the clearance procedures of the Office of ?!anagement and Budget as required by the Paperwork Reductien Act of 1980, PL 96-511.
