The NRC Staff bears some of the responsibility for the cheating which occurred at TMI.
They have continued to administer ' routine' examinations despite warnings and recommendations by the Kemeny Commission.
They administered the examinations in a manner,that facilitated cheating.
They failed to provide a system to prevent and detect chnating.

The NRC Staff constrained their investigation of the extent of cheating at TMI.
They permitted the Licensee to influence the investigation o f cheating.
3.
Licensee man'agement had knowledge of, encouraged and showed negligent failure to prevent cheating.
They were aware of cheating prior to the investigations by the NRC.
Licensee in a manner which facilitated cheating.
