Dear Mr. Emigh:

This is in partial response to your letter of August 27, 1981 in which you requested, pursuant to the Freedom of Information Act, copies of all reports of misadministrations of radioactive materials reported by and/or due to actions of nuclear pharmacies.

In a previous F0IA request identified as F0IA-81-100 (ICE), copies of NRC documents containing similar information regarding nuclear pharmacies were placed in the NRC Public Document Room (PDR),1717 H Street, N.W.,

Washington, DC 20555, where they are available for public inspection and copying. The charge for reproducing records located in the PDR is five cents ($0.05) per page, as specified in 10 CFR 9.14(a) of the Commission's regulations.

Copies of these documents can be purchased by writing directly to the PDR. The number of pages per document is indicated on the attachments to our response letters dated April 10, June 2, and June 10, 1981, copies of which are enclosed.

Upon your agreement to pay the reproduction charges, the PDR will arrange for the records to be reproduced by Literature Research Company, a private reproduction contractor servicing the PDR. You will be billed by Literature Research Company for the reproduction chargas, plus tax and postage.

Please find enclosed the two documents listed on Appendix A.

You agreed, in a telephone conversation with Mr. Ron Smith of the Office of Inspector and Auditor, to the deletion of the enclosures to both documents and names of individuals in document one in order to protect their privacy.

We are awaiting additional documents from another NRC office. As soon as we receive those documents, we will contact you.
