Dear Mr. O'Reilly:
01 nly 28, 1981, Mr. Virgil Brownlee of NRC Regial T.I was notified of a potmtial significant deficielcy or potmtial shstantial safety hrard calcerning an ITE interrupter switch, at June 29, 1981 an Interim Report was seit to Mr. O'Reilly.
Initially this item was idestified as a problem with m interrupter switch. 'Ihe actual problen was or a transfer switch which is similar to the interrupter switches. 'Ihe switches are described in the manufaturer's literature as interrupter switches.
SCE&G Nuclear Ehgineering has reviewed the problem md cmsideral both Brown Boveri Electric, Inc. recommdatiois and SCE&G operatims nnintmance ocncerns. 'Ihe conclusions of the review is that the above iten is a significant deficielcy under 10CFR50.55(e). 'Ihe details of the item are attached. '1his is cmsidered a final report of the nutter.

che blade of the ITE switch failed to close, thus cnly two phases of power were fed to the 3 phase Pump notor. %e motor failed to start, thus me of the three Charging / Safety Injection Pumps was out of service. Ioss of the marging/ Safety Injecticn Pump oculd result in failure of the safety injecticn systen if another single failure were to occur. Inspecticn of the connecticns to the switch blades revealed that insufficielt targue cn the holding nut md misalignmatt of the min switch blade caused the problem.
