The Brown Boveri Electric, Incorporated, Switchgear Systems Division's corrective action response letter dated April 8,1981, states in part:
"All personnel performing final test functions will report directly to the Quality Assurance Department during the performance of those functions when Certificates of Canformance are required.
"The enclosed organization chart shows this change.
This change be-came effective April 1,1981."
The enclosed organization chart depicts a bloc'K identified as:
Final Test, reporting to the QA Manager Contrary to the above, the revised organization chart had not become effective April 1,1981; as evidenced by an undated organization chart incorporated into the QA Manual and QA Procedure.
The incorporated organization chart did not depict a block identified as " Final Test," such as was depicted in the chart provided with the corrective action response letter.
