the Applicable Code Gentientn:

Mr. M. Siegel orally reported a Potential Reportable Deficiency to Mr. Glenn Walton, NDC, Region I, Hesident Inspector, Beaver Valley Unit #2 Project, on Decerrber 11, 1981. 'lhe Ictential Significant Deficiency, SDR 981-05, concerned the use of two welders who were not qualified for the thickness of the welas involved. Ike of the af fected w +1s were in Line 2-CliS-004-72-2, anc three are in Line 2-SIS-004-96-2.

'1he wulders were qualified to weld up to a nuterial thickness of 0.436".

The welos in question had a nominal wall thickness of 0.531".

The nuxinum thickness trom actual neasurenent was established as 0.536".

Our inneuiate action was to initiate a Nonconformance ana Disposition Report,

  1. 6444, arxl institute an investigation to establish the scope et the prublem.

T he investigation incluceu a revicw at 11/ cata sheets previously acceptea by the Instectors ~ho were involvea in the affectea welds.

In aadition, a further 17 data sheets randomly selected were examincti.

1> rom this review, no other urx3ualified welders wtre found to be assigned to perform welding activities on specitic welds.

'1he construction forces reviewed our Weld Analysis Report and approxinutely 500 welu data sheets, and also established that no acaitional violations had occurred.

Fn.m this review, we cane to the conclusion that the problem is an isolated ine'eert.

A further investigation wa< iun tated to establish how this indicent had occurred.

.riewed, and it appears that they nude certain assumptions regarding the qumiifications ot the two welders involvea, as the two welaers lud been performing similar welding in the area, anu the slight increase in wall thickness was tut noticed. They have been instructed to always reter to the Welaer's Qualitication Record, which is available to them, when performing Attribute ML-MS-004 of our Inspection Plan 7.2, that requires the welder's 8201260292 820112 PDR ADOCK 05000412 S

qualifications to be verified at the fit-up stage. In addition, we have nade the Welder's Qualification liistory Iog nore easily available to the Quality Control Inspectors.

'Ihe construction forces have re-trained the welding supervision, re-emphasizing the need for conplete conpliance to our welding program.

All of the welds involved had been subject to radiographic examination on conpletion. These radiographs were re-examined against the raIuirements of

/ME Section III and ASME Section 1X (Welder's Qualification Standard), and found to be acceptable in leth instances at the first time of submittal, itxlicative of high quality welders being involvod. In addition, both welders were re-tested to the required thickness, and also were found to be acceptable at the first time of sulsnittal.

The original incident described by N&D #6444 has been evaluated by our Engineers and is tot considered to have a safety iIrplication. The Potential ReIortable Deficiency has therefore been determined as not to be reix)rtable.
