0 7/17/81 1.0 P_URPOSE This procedure describes the actions taken prior to possible activiation of the IELP Corporate Emergency Response Plan, in response I
to a declared Emergency Action Level, at the DAEC.
2.0 APPLICABILITY Ihis procedure applies to the Director - Nuclear Generation (or alternate) who is responsible for activation of the IELP Corporate Emergency Response Plan.
l l 3.0 RESPONSIBILITIES (G_/
3.1 Director - Nuclear Generation (or Alternate)
Determine appropriate course of action for a notification of an Unusual Event or an Alert.
Ensure activation of the IELP Corporate Emergency Response Plan for a Site or General emergency.
4.0 INSTRUCTIONS 4.1 Upon notification of an Unusual Event or an Alert, the Director -
Nuclear Generation shall initiate, at his discretion, one of the following courses of action:
f 4.1.1 Inform appropriate IELP personnel of the situation at the (m) DAEC. (Use Attachment 1 as a guide) .

Standby does not constitute activation of the Corporate Emergency Response Plan. Those individuals placed on standby have no further action, other than to keep apprcpriate IELP rronagement apprised of their location.
4.1.3 Activate the Corporate Emergency Response Plan.
4.2 Upon notification of a Site or General Emergency, the Director, Nuclear Generation initiates immediate activation of the Corporate Emergency Response Plan. Simultaneously, with activation, the Director, Nuclear Generation assumes the position of Emergency Response and Recovery Director. -
4.3 Activation of the Corporate Emergency Response Plan implements CPIPs 1.2 through 3.2

Upon activation of the Corporate Emergency Response Plan (The Plan),
this procedure identifies individuals assigned key roles in the Corporate Emergency Response Organization and describes their notification and work assignment.

2.0 APPLICABI LITY l
Upon activation of The Plan, this procedure applies to Corporate anergency Pesponse Organization personnel.
3.0 RESPONSIBI LITIES 3.1 Emergency Response and Recovery Director Initiate and coordinate the notification process.
Communicate and coordinate with the Emergency Coordinator to apply corporate manpower and resources as necessary.
Ensure notification of appropriate personnel.
(Attachment 3 provides a listing of all corporate personnel available for response).
3.2 Emergency support Manager Ensure notification of appropriate personnel.
Coordinate with the Radiological and EOF Manager to assign personnel to use the status boards.

(_s) v Ogg II AND NOTIFICATION Reds on Date - 8710/8y 4.0 INSTRUCTIONS 4.1 The Emergency Response and Recovery Director shall initiate notification of key individuals of the Corporate Daergency Response Organization as follows:
4.1.1 Determine individuals to be contacted.
The particular individuals to be contacted within the Corporate Energency Response Organization will depend upon the event and classification. Attachment 1, "Emerg ency Response Organization Mobilization Matrix," is to be used as g-) guidance to identify those individuals. The matrix is used as
() follows:
- a. Identify Emergency Action Level (obtained from DADC) .
- b. Use matrix to determine individuals to be contacted (blank box indicates no centact) .
- c. Determine required action to be included in message (from symbol legend) .
4.1.2 Oantact individuals.
The primary means for notification should be through the commercial telephone system. Alternately, remote beepers (pagers) are carried by individuals as shown in the l attachment s. The Duers* ncy Response and Recovery Director or his designee shall conduct the initial notifications.

- a. The general, overall flow of communication is l l
illustrated in Attachment 2. Arrows represent specific i lines of communication.
NCTfE The Dnergency Response and Recovery Director initiates several " chains" of notification which should occur in parallel.
- b. Attachment 3 is a combination telephone list and communication log and should be used to identify personnel avaliable, and record when notification takes place.
- c. Allow at least 10 rings (about 1 minute) for a call to be answered. If there is no response, move on to the alternate contact.
- d. If, for any reason, a primary and an alternate cannot be reached for a position which is in turn responsible for further contacts, the individual attempting contact shall assume responsibility for those succeeding notifications. For example, if the Energency Response and Recovery Director cannot reach the Radiological and EOF Manager or his alternate, the anergency Response and Recovery Director shall assume the notification responsibilities of the Radiological and EOF bhnager.
In addition, his designee shall continue efforts to reach the primary and/or alternate contacts until notification is completed.

4.2 tbtification message content should be prepared as follows:
4.2.1 Message content should be determined based on Attachment 1.
Attachment 4 should be used for standby status noti ficati on.

G 4.3.1 Each individual with notification responsibilities should maintain a (bmmunications Pog Sheet (Attachment 3) .
4.3.2 Temporary reassignment of responsibilities may be necessary if a primary and an alternate contact for an emergency response organization position is initially unavailable.
4.3.3 The hrgency Response and Recovery Director or his designee shall monitor the status of communications periodically until notification is complete and all personnel are at their assigned locations. The Emergency Response and Recovery Director or his designee shall contact individuals with communication responsibilities to ascertain the status of their Cbmmunication Log and determine problem areas.' the 14th floor of the IE Tower (see Attachment 1) .
2.0 APPLICABILITY Upon activation of The Plan this procedure applies to Corporate Emergency Response Organization personnel involved with activation of the Emergency Operations Facility.
3.0 RESPONSIBILITIES 3.1 Emergency Response and Recovery Director Authorize Emergency Operations Facility activation.
Determine level of Emergency Operations Facility activation.
3.2 Radiological and EOF Manager Supervise and coordinate Emergency Operations Facility activation.
Coordinate with Emergency Support Manager to assign personnel to use the status boards.

4.0 INSTRUCTIONS 4.1 The Emergency Operations Facility shall be activated upon activation of The Plan. (See CPIP 1.1 " Corporate Emergency Response Plan Activation") .
4.1.1 The Radiological and EOF Manager will then assume responsibility for carrying out or directing specific activities required to activate the Emergency Operations

Facility. These activities shall include the following:
- a. Complete the Radiological and EOF Manager's Emergency Checklist (see Attachment 4).
- b. Complete the required emergency response organization notifications in accordance with CPIP 1.2, " Personnel Assignment and Notification."
- c. Report as soon as pocsible to the Emergency Operations Fac,ility.
4.1.2 The Support Services Coordinator shall coordinate the work required to physically prepare the Emergency Operations Facility area for activation. This shall include the following:

- Working level personnel from the Office of Disaster Services - This communication shall be for the purpose of providing information regarding radiological releases and radiological monitoring activities in the vicinity of the DAEC.
'Ihe Radiological Assessment Coordinator shall establish communications with offsite radiological monitoring teams after they are dispatched from DAEC (see EPIP 3.2, "Offsite Radiological Monitoring"). In addition, he shall initiate tracking of the plume on 10-mile and 50-mile radius EPZ maps, as applicable.
NOTE Release of any information to of fsite agencies other than that provided by the status boards shall require authorization by the Emergency Response and Recovery Director or the Radiological and EOF Manager only.
- b. Technical Support Engineering support is provided through the Emergency Operations Facility at the request of DAEC in the form of analysis, evaluation or design modifications. Refer to CPIP 2.2, " Technical Support Coordination" for details regarding specific procedures.

4.5.2 During normal working hours, direct IELP personnel who normally work on the 6th floor to evacuate the area.

Inform the Cedar Rapids Police Department of possible traffic congestion at the IE 'Ibwer due to TV trucks, lights, etc.
4.7 Deactivation 4.7.1 The Emergency Response and Recovery Director shall dire't the Dnergency News Center Director to deactivate the 4 V Dnergency News Center under the following conditions:
- a. The plant has been placed in safe and stable condition.
- b. Media interest diminished and full facilities are no longer necessary.
4.7.2 Dnergency News Center Director shall direct deactivation of the Duergency News Center as follows:
- a. Instructs the Support Services Coordinator to rumove extra communications equipment and office equipment and supplies. Attachment 2, " Equipment and Supplies Checklist" should be usad as a guide,
- b. Issue follow-up information to the news media (if necessary) from the lith floor of the IE ibwer.

0 7/17/81 4.1.2 After obtaining all pertinent information concerning the event, the Radiological Assessment Coordinator shall assume responsibility for making all subsequent dose calculations. The Site Radiation Protection Coordinator will continue to provide updated information to the RAC as it becomes available.
4.2 Initial assessment of radiological impacts to public shall be made as follows:
4.2.1 The Shift Supervising Engineer will make an evaluation to determine if a release to the environment has occured or is likely to occur. This evaluation shall follow the
() guidelines of EPIP 1.1, " Determination of the EAL" and be based on but not limited to the following:
- a. Noble Gas and Radioiodine Effluent monitor response.
- b. Area Radiation monitor response,
- c. Reactor Building, Turbine Building and Offgas Stack Sampling System data.
- d. Portable Air Sampling data.

4.2.2 The Site Radiation Protection Coordinator will periodically reevaluate information concerning the plant status until e stable condition is reached. The Radiological Assessment Coordinator shall be kept updated on the status.
4.2.3 If a release has occurred, Noble Gas and Radioiodine dose calculations shall be performed as outlined in EPIP 3.3.
The following should be calculated, as appropriates
- a. Maximum Concentration Location. This will be updated
() at least hourly to reflect any changes in wind speed and direction. Concentrations at other locations will be determined as necessary.
- b. Noble Gas and Iodine Dose Rates,
- c. Projected Noble Gas and Iodine Doses. This shall be determined for areas or locations where individuals are likely to accumulate significant doses approaching the Protective Action Guides outlined in Attachment 1.
Projected Dose calculations shall be updated at least every four (4) hours.

4.2.4 The Radiological Assessment Coo 6dinator shall assess the radiological impact to the public and provide appropriate protective action recommendations to the Radiological and EOF Manager. The basis for such decision-making should rest upon the following considerations:
- a. Current and projected plant release rate data.
- b. Meteorological conditions including expected weather changes as provided by the National Weather Service.
- c. Prevailing plant status and prognosis for change.
- d. PAG's and the likelihood of exceeding the guidelines.

These instances must be documer.ted by each responsible manager such that the activity or event can be reviewed and corrected following the conclusion of the emergency situation. All activities occurring during and directly after the emergency shall be considered " nonconforming" until disposition is determined through post-emergency review.

( 4.0 INSTRUCTIONS 4.1 Initial actions following a DAEC emergency shall include, but are not limited to, the following:
4.1.1 Upon activation of the Corporate Emergency Response Plan, the Safety Director will be notified of the emergency as outlined in CPIP 1.2, " Personnel Assignment and Notification."
4.1.2 The Safety Director upon his arrival at the IE Tower shall be informed of the number and extent of injuries including overexposures and personnel contamination and fatalities as a result of the declared EAL at DAEC.

4.1.4 The Safety Director or his designee shall notify the Safety Staff, as necessary.
4.2 Follow-up activities include, but are not limited to, the following:

This form should be filled out prior to contacting a Federal, State, or local official, and should be sequentially numbered.
4.1.2 After contacting an official the completed Emergency Update Message Form shall be filed in the communications log book.
. The log book serves two purposes.
- Provides a location to file the completed Emergency Update Message Form.

The log book will be maintained under the direction of the Emergency Response and Recovery Director.
4.1.3 When the condition or situation has been resolved:
- a. A Closecut Message Form (Use Attachment 3 as a guide) should be completed with all pertinent data. The Closeout Message Form includes a summary sheet which should be filled cut for each verbal communication that
() has been conducted during the event.
- b. Once the appropriate individual has been contacted, the completed Closeout Message shall be filed in the communications log book.
4.2 Upon activation of The Plan the following actions shall be initiated:
4.2.1 Immediate Actions
- a. The Emergency Response and Recovery Director should inform the following individuals as to the nature of the event, the status of the plant and potential consequences to the public in addition to notifications made in accordance with the DAEC Emergency Plan.

- a. News Release News releases are used for both initial and follow-up announcements to the media regarding an emergency situation. As a minimum, they shall be issued following activation of The Plan and when the plant has resumed normal operation. 4.1.1 Each exercise shall simulate an emergency that could result in offsite radiological releases which would require response by offsite authorities. Exercises shall be varied from year to year to meet the following additional criteria:
- a. Scenarios shall be varied from year to year such that all major elements of the DAEC Emergency Plan and the IELP Corporate Emergency Response Plan and their preparedness organizations are tested within a five-year period (see Attachment 3) .
- b. One exercise shall be initiated between 6:00 p.m. and midnight and another between midnight and 6:00 a.m.

4.1.3 Exercises shall be staged in as realistic a manner as possible.

- b. IELP participants should not be notified in advance
(') except where necessary to assure that the exercise will not cause injuries or prcperty damage. Offsite agencies will be notified in advance to coordinate the exercise; however, IELP shall request that the individuals participating from offsite agencies not receive advance notification,

Participants responsible for preparing areas such as the Technical Support Center and Emergency Operations Facility for use during activation, shall identify where the necessary equipment is stored or located and simulate equipment transfer and setup. This simulated activity shall identify the necessary manpower and the time required to fully activate the area.

- d. Participants shall perform all functions as though the exercise were an actual emergency, including start-up of equipment. Where these actions disrupt the operation of the plant, simulated actions shall be reported to exercise observers when possible.
4.2 The exercise shall be coordinated with representatives of IELP/DAEC and offsite support agencies including Federal, State and local government agencies as necessary.
4.2.1 The number of representatives contacted shall be consistent with the scope of the exercise to be conducted.
) 4.2.2 Each exercise shall include a requirement for support assistance from State and county agencies. Such assistance should include, participation by county emergency operation centers, local law enforcement agencies, and medical personnel (ambulance and Mercy Hospital in Cedar ,g, aapids). <
4.2.3 Federal agencies shall participate in exercises at least once every five years.
4.3 The Director, Nuclear Generation shall initiate the exercise and inform the Chief Engineer to proceed s..h the exercise and inform the Shift Supervising Engineer of the existence of a pre-established hypothetical emergency condition.

0 7/17/81 4.4 The Shif t Supervising Engineer shall activate the DAEC Emergency Plan by announcing the hypothetical emergency condition over the public address system.
NOTE The Shif t Supervising Engineer shall, at the beginning and at the end of the announcement, state, "This is an exercise, I repeat, this is an exercise."
4.5 All notifications of off-duty personnel and offsite support agencies shall include the statement at the beginning and at the end of the notification, "This is an exercise."

4.6 Each exercise shall include observers who will be located at areas where key emergency activities are conducted.
4.6.1 The number of observers scheduled will be dictated by the scope of the exercise.
4.6.2 Attachment 1 describes the observation areas for all key emergency activities at DAEC and the IE Tower, the major categories to be obs'arved, and the applicable emergency procedures. Each observer shall be familiar with the information which applies to his observation post.
4.6.3 Observers shall be stationed at predetermined locations for recording pertinent observations.

4.9 A review and pres 2ntation of exercise critiques shall be scheduled by the Emergency Planning Coordinator as soon as practical after the conclusion of the exercise.
4.9.1 Participating offsite agencies shall be invited to attend the review session and provide their own critiques.
4.9.2 Observer's Log Sheets and critiques shall be submitted to the Emergency Planning Coordinator.
4.9.3 Recommendations shall be made to improve operations.

g CORPORATE PLAN IMPLEMENTING PROCEDURE CPIP - 4 4 Page 1 of 5 PERIODIC PLAN AND gg V IGggE PROCEDURE REVIEW Re v hilon Date - 6/29/81 0 1.0 PURPOSE This procedure describes the means for reviewing, auditing, and updating IELP emergency plans ar.d corporate procedures. 2.0 APPLICABILITY This procedure applies to organizations and individuals assigned to review, audit, or update IELP emergency plans and corporate procedures. 3.0 RESPONSIBILITIES 3.1 Emergency Planning Coordinator O
- Ensure departmental reviews of the emergency response program, as required Follow organizational, regulatory or other changes which might impact The Plan or procedures Ensure that review and audit findings are evaluated and corrections are incorporated into the emergency response program Document the completion of reviews and audits of the emergency response program.

19ll PROCEDURE REVIEW Revision 0 Date - 6/29/81 4.0 INSTRUCTIONS 4.1 Reviews of the IELP emergency response program shall be conducted by those IELP personnel responsible for its implementation and may result in recommeded changes to The Plans and procedures. As a minimum, these reviews shall include the following: 4.1.1 A quarterly update of notification lists as follows:
- a. Replace the names and titles of terminated employees with those of new employees.
- b. Verify all existing telephone numbers and make the
() necessary changes. 4.1.2 Orills and exercises conducted in accordance with CPIP 4.2,
" Exercises" CPIP 4.3, " Training and Drills" and EPIP 6.2. " Training and Drills". These activities, along with recommended changes, shall be documented and incorporated into The Plan and procudures during the annual review process.
4.1.3 Audits shall be performed in accordance with Nuclear Generation Division Project Instructions " Safety Committee Audit Procedure."

Following receipt of the audit report from the Saf ety mmmittee, the Energency Planning Cbordinator shall document the audit completion and submit a written report to corporate and DAEC management and involved Federal, State, and local organizations. NOTE Audit reports shall be retained for 5 years. 4.1.4 Recommended changes to the emergency response program also may be identified by a review of the following:
- a. Organizational changes within IELP or support groups
- b. Changes in regulatory-requirements l

E CORPORATE PLAN IMPLEMENTING PROCEDURE CPIP - ' 4 3 Page E PERIODIC PLAN AF9 0' g PROCEDURE REVIEW Revision6 /29/81 Date 4.2 Recommended changes to the emergency response program as a result of the review process shall be implemented as follows: 4.2.1 Individuals responsible for review of a specific portion of the emergency response program, shall document the review I and recommend changes to the program in a report to the Dnergency Planning Coordinator. 4.2.2 Upon receipt of the review report, the Duergency Planning coordinator shall take the following actions:
- a. Obtain a licensing and technical review and approval of proposed changes.

- b. Approve proposed changes. ,

- c. Determine the impact of proposed changes to other areas of the emergency response program and ensure the implementation of these changes as necessary.
- d. Disure that changes are incorporated into the emergency response program.
4.2.3 The N2 clear Generation Division shall prepare and distribute changes to the emergency response plans and procedures as follows:
- a. Revised pages shall be dated and marked to show changes.
q b. An adjusted Table of Contents shall accompany each change distribution, as applicable.

- c. All changes shall show proper approval signatures prior to distribution.

- d. Distribution shall be to those IELP and outside I individuals and organizations indicated on the approved distribution list for IELP emergency response plans and procedures. Additional distribution shall be approved by the Daergency Planning Cbordinator.
- c. Copies of changes to the EPIPs and CPIPs shall be submitted to the NRC as indicated in 10CFR50, Appendix E.
