Dear Pz. Haynes:

PP&L is in receipt of a Cooper Energy Services (CES) letter dated November 23, 1981 (F.B. Stolba to NRC I&E) in which the subject deficiency was identified as a condition reportable under 10 CFR Part 21.

PP&L has reviewed this letter and concurs with CES's conclusion.

PPL'u's review of the subject deficiency leads to the conclusion that the subject deficiency is al so reportable under the provisions of 10 CFR 50.55(e). This was reported to Mr. L. Narrow (NRC Region I) by Mr. A. R.

Sabol, Manager-Nuclear Quality Assurance, by telephone on December 12, 1981.

The deficiency relates to the failure of a fuel injection nozzle seating surface on an emergency diesel generator head assembly supplied to Susquehanna SES by CES.

The attachment to this letter is a final report pursuant to the provisions of 10 CFR 50.55(e) containing a description of the problem, its cause, safety implications, and the corrective action.

We trust the commission will find the information forwarded by this letter to be satisfactory.

' SSES PLA-986 100450/100508 821-10 ATTACHm'T TO PLA-986 SUBJECT Failure of the fuel injector nozzle seating surface on "B" emergency diesel' generator head assembly.

DESCRIPTION OF PROBLEM PP&L NCR 81-435 documented the condition where a copper " bushing" was inadvertently extracted from the "B" diesel head assembly during removal of fuel injector nozzle 2-R.

CAUSE OF DEFICIENCY Cooper Energy Services stated in their letter of 11/23/81 to the NRC that the Fuel Injector nozzle hole was drilled larger than the specified 13/16" during manufacture and was repaired by Dalic copper plating the hole to proper size. It was discovered after performance of a 300 start test of the unit at SSES, that this plating failed due to the large difference between the coeffic'ient's of expansion between the copper plating and the parent metal, and the large temperature fluctuations that occur in this part of the head.

This failure was originally reported via PP&L NCR 81-435 as an inadvertent extraction of a copper " bushing".

ANALYSIS OF SAFETY IMPLICATION The failure of the plating during diesel operation could cause the fuel injection nozzle to be blown out of the cylinder head and could result in a top side engine fire. Therefore, this design deficiency is considered significant and reportable under 10 CFR 50.55(e).

CORRECTIVE ACTION The head that failed has been replaced with a new head from Cooper Energy Services. Since CES has. stated that diesel generator set "B" is the only set delivered to SSES with this' defect in,the' head assembly fuel injector nozzle seating surface, corrective action is considered complete for Susquehanna.
