Attached is supplementary information referenced in FSAR Section 2.2.1.1 and used as a basis for the conclusions drawn therein.
This supplemental information is in the form of a three ring binder report entitled " Transport of Toxic Chemicals and Control Room Habitability Study". Along with that report there are two Acco Press Folders.
Folder No.1 is entitled " Completed Company Checklist" and Folder No. 2 is entitled "U.S. 52 - Truck Survey Daily Survey Sheet".
