Appendix' NOTICE OF VIOLATION Oaklawn Hospital License No. 21-17068-01 As a result of the inspection conducted on December 3, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the.following violation;was identified:
License Condition No. 12 states that licensed material shall be used by, or under the supervision of, Donna C. Crandell, M.D., or Merwyn M. Crandell, M.D.
Contrary to the above, since May 1, 1981, licensed material has been used by individuals not listed as authorized users and who have not been under the above individuals' supervision.
This is a Severity Level IV violation (Supplement VII).
