This refers to the routine safety inspections conducted by Mr. D. R. Gibbons of thic of fice and Mr. J. Nicolosi of our Region I of fice during the period of March 24, 1981 through December 8, 1981, of activities authorized by NRC Byproduct Material License No. 12-00622-07 at the follo"ing locations:
Madison Heights, Michigan; Perry, Cincinnati and Cleveland, Ohio; Pittsburgh and Monongahela, Pennsylvania, and Milwaukee, Wisconsin and to the discussion of ot.r findings with members of your staff at the conclusion of each of the inspections, and to a telephone conversation between Mr. D. R. Gibbons and Mr. E. Banfield on December 17, 1981.
The inspections were an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tions consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
In addition to the abeve areas, the inspectors examined actions described in your letter dated September 27, 1979, regarding apparent items of non-compliance found during inspection of your facilities at Bridgeville, Pittsburgh and Trevose, Pennsylvania; Milwaukee and Pleasant Praire, Wisconsin; Cleveland, Ohio; Roseville, Minnesota; and Hancocks Bridge, New Jersey. Our inspectors also examined action described in your letter dated July 29, 1980, regarding items of noncompliance found during our July 2, 3 and 25, 1980 investigation at your Springdale, Ohio facility.
We have no further questions regarding these matters.
During these inspections, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in the enclosed Appendix.
A written response, submitted under oath or affirmation, is required.
