Dear Mr. Collins:
This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period October 1, 1981 to October 13, 1981.
Th, following response to the items contained in the Notice of Violation is hereby submitted:

" Procedures, Administrative Controle,"
states in part that, ".
. written procedures shall be established, implemented, and maintained.
Administrative Procedure P-2, " Equipment Clearances and Operation Deviations," Issue 4, dated June 12, 1981, Section 4.1 Step 15, states in part that, "...The Standard Clearance Points form is checked of f as tags are pulled and equipment is returned to normal operation
." Procedure OPOP I, Section B determines the normal operability of certain valves via a Sealed Valve Checklist which states that V-22184 should be sealed open.
In accordance with OPOP III, V-22184 must be scaled open whenever reactor power is greater than 2%.
Contrary to the above, on October 20, 1981, at 2:20 p.m. MDT, with reactor power at 34%, the NRC inspector determined that V-22184, Loop I Main Steam Electromatic Relief Valve Shutoff Valve, was not returned to normal due to the valve not being sealed.

-(l) The corrective steps which have been taken and the results achieved:
a) The valve was immediately rescaled upon notification by the NRC inspector.
b) Another step has been added to our clearance process. This step includes stamping a clearance after the tags have.been collected to indicate: 1) Were there any sealed valves on the clearance % and 2) Were these valves properly resealed?
This will remind the operator of the importance of the sealed valves and his need to reseal them. The memo sent to the operators concerning this step is provided as Attachment A.
(2) Corrective steps which will be taken to avoid further violations:
The standard clearances are presently being put into our compu.ter base record keeping system. The form used for these clearance's has been designed to include a column designating whether any of the valves on the clearance are sealed valves and a -statement verifying the valves were properly resealed. An example of this format is included as Attachment B.
This program will then replace the stamp discussed above.
(3) The date when full compliance will be achieved:
Full compliance regarding the specific violation of not sealing V-22184 was accomplished on October 20, 1981. The procedure to enhance our complying with the sealed valve requirement was implemented on November 12 and will continue until the standard clearances are input irto the computer. This is now expected to be completed by Maren, 1982.
B.
Procedure HPP-20, " Operation and calibration of Radiation Detection Instruments," Issue 12 dated September 9, 1981, states in part under the section titled " Precautions", ".
.Do not leave calibration source unattended outside the source storage room.
Contrary to the above, on October 30, 1981, at 9:00 a.m. MST, the NRC Inspector determined that sources numbered 184 and 92 were left unattended outside of their assigned storage room.
This is a Severity Level V violation (Supplement I. E.)
(1) The Corrective steps which have been taken and the results achieved:
The Health Physics Technician involved was reminded of the requirements of HPP-20 with respect to the handling and accountability of radioactive sources.

(2) Corrective steps which will be taken to avoid further violations:
Order #81-25 was placed in the Health Physics Order Book on October 30, 1981, spelling out Fort St. Vrain radioactive source handling requirements. The order was reviewed and signed off by all Health Physics Technicians.
(3) The date when full compliance will be achieved:
October 30, 1981.
l It should be noted that sources number 184 and 92 were in locked shielded containers at the time of the occurrence.

NRC VIOLATIONS k'e have recently received two violations from the NRC about failure to follow pro-cedure because we have not rescaled valves.
These occurred when clearances were picked up and the operator failed to rescal the valves.
It's imperative that we take some step to eliminate any future situations like this.
A stamp is now provided near the clearance log that should be stamped at the bottom of every clearance when it's returned.
It is then the responsibility of the person returning the clearance to check the scaled valve list for the following:
1) to see if any of the valves on the clearance are on the sealed valve list, and 2) if there are sealed valves, see that they have been properly rescaled. The operator should initial these statements.
It is important that everyone understsud the procedural significant of these scaled valves and the need to provide assurances that these valves are always properly positioned.
