Pursuant to STS paragraph 3.7.9.a the following information is provided:

While performing an IC-month inspection of penetration fire barriers (Trojan MP-12-13) it was discovered that some of the penetrations were either marginal, questionable, or not functional as 3-hour barriers. Upon receiving this information we followed the action statement as required.

Smoke detectors were verified operable and a roving fire watch was required to inspect all affected areas hourly.

This special report is being written to the commission pursuant to specifi-cation 6.9.2 outlining:

plans and schedule for restoring the fire barriers to functional status.

ACTION TAKEN:

Smoke detectors were ver.fied operable by the Operations group and a fire patrol was initiated to :nspect the affected areas horrly.

CAUSE:

Breaches were caused by the following:

1.

New penetrations not completed 2.

Damage by workmen working in area 3.

Grouting cracked or chipped 4.

Removal of existing conduit or cable from a 3-hour fire rated wall NOTE: Many of the penetrations had nicks and gouges less than 1/4" long.
