APPENDIX A NOTICE OF VIOLATION Halifax Community Hospital License No. 45-18401-01 As a result of the inspection conducted on October 14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following
. violations were identified.
A.
License Condition 16 requires in part that licensed material be possessed and used in accordance with statements, representations 'and procedures contained. in your application dated December 22, 1978. One procedure in your application states that areas used for elution of Molybdenum.99/
Technetium 99m generators and preparation of individual patient doses will be surveyed for contamination daily.
Contrary to the above, the requirement to survey dose preparation areas.on a daily basis was not met in that as of the date~of the inspection, no daily surveys of the elution and dose preparation areas have-been performed.
l This is a Severity Level IV Violation (Supplement IV.D.1).
B.
License-Condition 16 requires in part that licensed material be passessed and used _in accordance with statements, representations, and procedures
. contained in your application dated December 22, 1978.
One procedure pertair.ing to-instrument calibration. states that survey instruments will-be calibrated quarterly and batteries will be _ checked and replaced as necessary.
Contrary to the above, the requirements to calibrate your survey instruments quarterly'and perform battery checks were not met in that on the day 'of the inspection, the Victoreen Cutie Pie survey meter was overdue for calibration (due date 8/81) and furthermore a battery check indicated that the batterias in the instrument needed replacing.
This is a Severity Level IV Violation (Supplement VII.D.2).
