6 Gentlemen:
On September 30, at approximately 7:20 a.m., a truck carrying sixty packages containing low-enriched uranium dioxide powder, (Certificate of Compliance Number USA /5768/AF, Model Number BB-250-2) and twenty-four empty 55 gallon drums overturned at the junction of the Lynchburg Express-way and Candlers Mt. Road in Lynchburg, Virginia. The Tri-State Motor Transit Company truck was en-route to the Babcock 6 Wilcox Company, Com-mercial Nuclear Fuel Plant (CNFP) from the Babcock 6 Wilcox Company, Apollo, Pennsylvania Plant. The containers were thrown off the truck onto the median strip within approximately fifty feet of the truck bed.
Local police contacted B6W shortly after the accident and the first B6W officials arrived between 7:30 and 7:35 a.m.
Also, the first CNFP response team arrived at the scene around 7:35 a.m.
By approximately 9:25 a.m., all containers, the tractor trailer, and the accident scene had been surveyed, and subsequently released for recovery operations.
Surveys showed that no radioactive material was released from any of the BB-250 containers or the empty drums. All containers were removed to the CNFP for unloading or storage. The recovery operations were completed, with a final survey of the accident scene rnde around 4 p.m.

Damage to the BB-250's consisted largely of dents in the outer container.
Nine BB-250's experienced minor reduction of outer containment integrity where a small section of the outer container was pushed in and/or down from the drum lid and ring, although no BB-250 lost its drum lid or ring.
While one BB-250 was found to have a tear in the outer container skin about twelve inches long by four inches wide at the widest point, this container lost less than 10% of its vermiculite.
For all sixty BB-250's, there was no damage to the inner container, with only minor repairs (e.g.,
straightening.a position ring) being required.
Each BB-250 contained four loaded metal cans, and all cans were found intact including their plastic inner liners. The outer containers of the BB-250's were damaged as described above and will be replaced. The inner packages maintained their integrity and will be retained for reinsta11ation in the replacement outer containers.
This letter is intended to satisfy the reporting requirements of 10 CFR 71.61 for the Commercial Nuclear Fuel Plant License SNM-1168 and the Apollo Plant License SNM-145.
