Dear Mr. Haynes:

On December 1, 1981, after making repairs to valves in the sodium hypochlorite injection system a Mechanic incorrectly opened a drain valve that allowed approximately 3850 gallons, 12 percent solution of sodium hypochlorite to discharge into the river. Most of the discharge was drawn into the circulating water pumps, passed through the condenser returning to the river via the discharge canal. Duration of the discharge is estimated to be approximately one hour.

Calcula-tions estimate that chlorine residual in the discharge canal reached a maximum of 10 P W.

This exceeded Environmental Technical Specifica-tions, Section 7.3.1.1 limit of 0.1 milligram per-liter.

Investigation of the discharge canal immediately following the discharge revealed an estimated 250 spottailed shiners and 50 other small fish were killed.

Maintenance personnel incorrectly attempted to realign the injection system following repairs to valves previously isolated by operating personnel.

All Maintenance personnel have been reminded not to perform operator functions.
