Dear Mr. Kline:

The subject investigation was held to review the status of the referenced subjects, and other items brought forth at the suggestion of Cleveland Electric Illuminating personnel.

Mr. Jack Kline November 30, 1981 Items to be Closed (Satisfactory Resolution)

We have reviewed the corrective action and agree with the resolution of the following items;

1. The socket welds on the lines leading to the control rod drive assembly appear satisfactory and acceptable as corrected. This item is now considered , closed.,
2. The D'ikker safety relief valves were improperly radiographed by the manufacturer. The corrective action taken in NRR-50-440/

80-05-01 and NRR 50-441/80-05-01, including the open item we had previously questioned now appears to have been satisfactorily corrected. The verification of the corrective action and explanation is acceptable.

It is our understanding this also complies with the corrective action required by the NRC vendor compliance inspector.

This is now considered to be a closed item.

Action Taken'to Date (On Continuing Items)

1. The radiographs of Pullman Power Piping welds which were apparently improperly shimmed were being reviewed and re-evaluated by properly qualified CEI personnel who had been qualified SNT-TC-1A Level III and Level II radiography.

Satisfactory demonstration of compliance was achieved by using the basic compliance criteria set forth in our letter of August 24, 1981.

(a) We reviewed the control radiographs on system 2-E12-188 AB Job 8405 6" diameter - 403 welds .280" vall "t" Original F-13922 Mark 2E 12 - G-RH, 188AB Satisfactory demonstration and acceptance of 1612 radiographs.

(b) We reviewed the control radiographs on system 2 E12 83AB i

4" diameter - 269 welds .237" wall "t" l Mark 2E12 G-RH-83-AA Satisfactory demonstration and acceptance of 1076 radiographs.

(c) We reviewed the control radiographs on system 2E12-109AB SWC

'8" diameter -250 welds .322" wall "t" Satisfactory demonstration and acceptance of 1000 radiographs

2. We therefore accept the radiographs of the 922 welds and 3688 radiographs represented by the above demonstration review.

1. Since we had asked for 100% re-review and proof of sensitivity at the beginning of the corrective action for all of the 10,000 radiographs, acceptance of the first 3,688 films still leaves over 6,000 radiographs which must be reviewed.

These are considered as open items until such time that satisfactory corrective action and acceptance is attained.

2. Control radiographs for 3 inch diameter welds have not been reviewed and accepted. These
  • should be available at our next visit to the plan't.

New Items

1. (6) penetrations to be embedded within the annulus concrete, necessitated due to redesign which will dampen vibrations due to SRV actuations.

(a) The current Code Case N-258 is not applicable in this case, and we certainly can't use a proposed revised Code case.

(b) These penetrations are a part of the containment, and this containment is being built as an Ohio special.

(c) In order for the above proposal to be acceptable it would be necessary to install leak chase channels, have all work including concrete portions to be inspected by your Authorized Nuclear Inspector and do all work in compliance with the rules set forth in Section III Div. 2, of the ASME Code. Upon completion of a corrective action plan, said plan should be submitted to the State of Ohio.

2. Circumferential T-stiffeners on outside of containment vessel made of webs 1 " thick with flanges 2" thick all made of P-1 material. The flanges are 16" wide, and include 10 bute welds which were not postweld heat treated in accordance with Section III, Div. 1 NE-4431.

It was furr.her understood that no authorized third party inspection had been made of these welds.
