BASES: 10 CFR Part 50, Appendix B and 10 CFR Part 21. B. SCOPE: This inspection was made as a result of: (1) the issuance of a 10 CFR Part 21 report by General Atomic Company concerning failures of Rockbestos RSS-6-104 coaxial cable during a steam line break test at 4210F and in oven testing of coils at 355 F while energized at 875V; and (2) a potential 10 CFR Part 50.55(e) report by Kansas Gas and Electric Company regarding nonconformance to specification requirements of Rockbestos cable delivered to Wolf Creek Generating Station, Unit 1 site. VIOLATION: Contrary to paragraph 21.6 of 10 CFR Part 21, the required posting of 10 CFR Part 21, Section 206 of the Energy Reorganization Act of 1974 and adapted precedures, or Section 206 and the permissible alternate notice, had not been accomplished in the 195 Church Street offices. B. NONCONFORMANCE: Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Appendix D, paragraph 2 of Tennessee Valley Authority contract 80KS-825730 (STRIDE), Nonconformance Report, Form TVA 10548 had not been used to report RSS-6-100 series coaxial cable nonconformances with disposition "use as is" or " repair to other than original specifications." C. UNRESOLVED ITEMS: None D. STATUS OF PREVIOUS INSPECTION FINDINGS: 1. (Closed) Unresolved Item (80-01): Qualification testing for cable insu-lation rework had not been performed for Grand Gulf site. Data was reviewed showing that the testing had been performed; however, the data had not been submitted to Bechtel. It was verified subsequent to the inspection that the formal report had been submitted to Bechtel on October 8, 1981. 2. (Closed) Follow-up Item (80-01): Assess the effectiveness of controls of the jacketing and rework (insulation and brazing) operations. No brazing is permitted on nuclear cable. All rework personnel have been certified through either the New Haven or the East Grandby Training Program. Nuclear cable can only be reworked when permitted by the purchaser. Insulation rework is tracable to a single conductor within a multi-conductor reel. Procedures for insulation rework were on the floor available to the rework personnel. 3. (Closed) Follow-up Item (80-01): Evaluation of the status of qualifica-tion testing related to insulation rework. San Onofre Qualification Testing was completed and accepted by Bechtel. E. OTHER FINDINGS OR C0ffiENTS: 1. Regional Requests a. A 10 CFR Part 21 report by General Atomic Company reported failure of Rockbestos RSS-6-104 coaxial cable during steam line break test 0 0 at 421 F and oven testing in coils at 355 F while energized at 875V. PAGE 3 of 3 and the braid, was caused by dielectric expansion and braid shrinkage which brought about kinks in the inner conductor. Rockbestos has redesigned the RSS-6-100 series cable mechanically and are performing qualification testing. They have contacted all purchasers of the cable and are replacin cable which could encounter adverse conditions. (See B. above b. Kansas Gas and Electric Company reported that Rockestos control cable had failed tensile strength and elongation tests. The nonconforming cable had been shipped to the Wolf Creek site with acceptable certified test reports. The cable had not been identi-fied as nonconforming during manufacturi7g; therefore, no Off Standard Reports (OSRs) or Bechtel Supp Mer Deviation Disposition Requests (SDDRs) were written to disposition the cable. To correct these deficiencies Rockbestos has: (1) written OSRs and SDDRs covering these specific deficiencies; (2) tested samples of the defective cable with acceptable results from all the tests; (3) evaluated the deficiency for 10 CFR Part 21 implications and determined that the cable could perform its safety-related function in spite of this deficiency; and (4) instructed personnel responsible for initiating OSRs and SDDRs to issue them for nonconforming wire and cable. 2. Implementation of 10 CFR Part 21 Reviewed quality procedures No. Q-27, Procedure For Reporting And Evaluating Deviations From Technical Reauirements Which Could Create a Substantial Safety Hazard in Nuclear Safety Related Cable, dated December 8, 1977, to verify that procedures had been adopted. Observed posting at Nicoll Street and Church Street locations. Posting in accord-ance with 10 CFR Part 21 was accomplished at Nicoll Street. (See A. above) 3. Follow-up Items a. Rockbestos reported a potential 10 CFR Part 21 condition to NRC Region I as a result of being advised by Stone and Webster that Shoreham Nuclear Power Station had received cable with indentations in the conductor insulation. These indentations have been caused by a spirally wrapped binder thread. This problem is presently under investigation by Rockbestos and will be evaluated during a future inspection. b. An evaluation of the original qualification testing of the RSS-6-100 series coaxial cable will be performed on a future inspection in order to determine the reasons for the disparity in results between the General Atomic Company and Rockbestos testing. i
