Dear Mr. Haynes:

On October 23, 1981, in accordance with 10CFR50.55 (e) , we re-ported verbally to Region 1 a deficiency with fire stops installed by Keasby-Bisco in floor penetration sleeves. This letter serves as our 30-day written report of this deficiency.

Description of Deficiency

During installation of fire stops and seals in the radwaste building, one (1) radiation resistant fire stop identified by the installing contractor, Keasby-Bisco, as type SF-130NH fell out of a spare floor penetration sleeve. A second fire stop of the same type was found to be slipping out of another spare floor sleeve. Application of this type of fire stop was discon-tinued while an investigation was performed to determine the cause of this deficiency.

The investigation determined that the sleeves from which fall-

} out/ slippage had occurred were painted with an epoxy formulated paint which prevented the fire stop material from achieving adequate " grip". Further investigation established that the majority of mechanical sleeves at Shoreham had been painted such that the deficiency might initially be considered to affect all 8112140160 911120 i PDR ADOCK 05000322 1

SNRC-637 November 20, 1981 Page 2 floor sleeves. However, an engineering evaluation has determin-ed that the problem is solely associated with the type SF-150NH ,

fire stop material.

There are three (3) basic fire stop materials utilized at Shore-ham which are designated by Keasby-Bisco as SF-20, SF-60 and SF-150NH. The SF-20 material is a Dow Corning product 6548 silicon foam with a cured density of 20 lbs/cu ft. This material has a very high capacity for expansion and will have no difficulty in generating the necessary grip with the painted sleeve for self support. The SF-60 material is a Dow Corning 2 part product Sylgard 170 A&B silicon elastomer with a density of 60 lbs/cu ft. This material has a very low capacity for expan-sion and is used in a very limited application where cable ampacity requirements dictate or where differential pressure levels exceed the ability of SF-20. It is normally applied to a depth of approximately 5 inches on installation and cannot fall out. The type SF-150NH is essentially the Sylgard product to which iron particles have been added to achieve the radiation resistance requirements. The end product consists of approxi-mately 34% of Sylgard 170A&B and 66% iron particles with a cured density of 150 lbs/cu ft. The high density in combination with low expansion capability and full floor depth application results in insufficient grip to the painted sleeve for self support, permitting the fire stop to fall out.

Corrective Action A support arrangement has been designed for the type SF-150NH fire stop and will be used for all new radiation resistant fire stops applied to floor sleeve penetrations. Typical examples of the support configuration and application table are provided for your information as enclosures 1 through 4. Those penetrations which have been identified as having this type of fire stop applied prior to discovery of the deficiency will be reworked to include adequate supports. This rework should be completed by April 1982.

Additional action which has been implemented is to perform qual-ification fire testing for fire stops and seals in painted penetrations. Although no difficulties are anticipated in achieving satisfactory test results, this testing was considered necessary for purposes of matching the as installed fire stop
