APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Dockets 50-317, 50-318 Calvert Cliffs Units One and Two Licenses DPR-53, DPR-69 As a result of the inspection conducted on October 22 - October 30, 1981 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

A.1 10 CFR 50 Appendix B Criterion XVI requires measures for promptly correcting conditions adverse to quality.

Contrary to the above, the FAIL-OPEN characteristic of Unit 1 Component Cooling Water (CCW) Containment Supply Valve 1-CC-3832-CV was not promptly changed to FAIL-SHUT: Field Change Request FCR-1089, Revision 2, originated September 2, 1976 to change valve 1-CC-3832-CV to FAIL-SHUT was not implemented until about 8 p.m., October 22, 1981.

This is a Severity Level IV Violation (Supplement I) applicable to DRP-53.

A.2 10 CFR 50 Appendix B Criterion XVI requires that quality correcting measures assure that corrective action is taken to preclude repetition.

FSAR Article 1C.16, Revision 38, April 30, 1975 states that the QA Department verifies that responsible departments have taken adequate corrective action to prevent recurrence of non-conformances. Quality Assurance Procedure QAP-26, Revision 24, July 28, 1886, Attachment A defines non-conformance as a deficiency rendering the quality of an item unacceptable or indeterminate.

Contrary to the above, measures to correct the FAIL-0 DEN characteristic of Unit 1 CCW Containment Supply Valve 1-CC-3832-CV did not include measures to prevent recurrence:

Field Change Request FCR-1089, Revision 2, originated September 2, 1976, was applicable to Unit One only and a problem causing corresponding Unit 2 Valve 2-CC-3832-CV to l

fail OPEN on loss of air or electrical power was not licensee recognized until about 2:50 a.m., October 23, 1981, after the NRC resident inspector informed the licensee about 11:30 p.m., October 22, 1981 that a problem existed with the failure made of Valve 2-CC-3832-CV.

This is a Severity Level IV Violation (Supplement I) applicable to DRP-53 and DPR-69.

A.3 10 CFR 50 Appendix B Criterion V requires appropriate drawings for activities affecting quality.

FSAR Article 1C.5, Revision 38, April 30, 1975 requires that drawings specify appropriate criteria for verifying satisfactory accomplishment of important activities.

Contrary to the above, the effective controlled electrical schematic for Component Cooling Water Containment Supply Valve 1-CC-3832-CV was not appropriate on October 22, 1981:

Valve 1-CC-3832-CV would fail SHUT on loss of electrical power (isolating safety functions from ron-safety functions), but Electrical Schematic lE-76, Sheet 14G, Revision 1, February 16, 1977 showed that the valve would fail GPEN.

This is a Severity Level V Violation (Supplement I) applicable to DPR-53.

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending the response time.

The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
