50-336 Millstone Unit 2 License No.: DPR-65 Hartford, Connecticut 06101 As a result of the inspection conducted on September 27 through October 31, 1981, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7,1980), the following violation was identified:

Technical Specification 6.8.1 states in part:

" Written procedures shall be established, implemented and maintained covering the activities referenced below:

a.

The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, November 1972.

b.

Refueling operations."

Regulatory Guic'e 1.33 dated November 3,1972, Appendix A, paragraph B requires, " General Plant Operating Procedures...

10. Preparation for Refueling...."

Station Procedure OP2209A " Refueling Operations", revision 6, dated 6-12-81 requires "Prcrequisites... all new fuel assemblies have been inspected in accordance with OP2210A, New Fuel Assembly and Control Element Assembly Receipt and Inspection."

Station Procedure OP2210A, "New Fuel Assembly and Control Element Assembly Receipt and Inspection", step 2.2 states in part:

"The following shall be verified to have a valid weight test before being used to handle fuel:

a.

Spent fuel cask crane main hook and auxiliary hook (15 ton) -

Ic3d test to 5 tons....

c.

Slings and cables to lift new fuel shipping container - load test to 5 tons....

d.

Slings and cables to handle shipping container hardware - load test to 3 tons. Sling and short fuel handling tool - load test to 1 ton...."

s Contrary to the above, on October 19, 1981, while conducting inspection of new fuel, completion of weight tests had not

[

been verified for the above-listed equipment. - Those slings, cables, and tools bore no unique identifying markings which could be correlated to records of prior testing.

This is a Severity Level V Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company would be required to submit to this office within twenty-five days of the date of this Notice, a written statement or _ explanation in reply, including:

(1) admission or denial of the alleged violations;-(2) the reasons for the item (s) of noncompliance if adnitted; (3) the corrective. steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance

will be achieved. However, your corrective action was taken during this inspection period and has been described and verified in the attached inspection report.
