r APPEllDIX A NOTICE OF VIOLATIO!!
As a result of the inspection conducted on October 13-15, 1981, and in accordance with the following enforcement policy, US FR 66754 (October 7,1980). The following violation was identified:
Requirements of the Technical Specification 6.3, " Written Procedures and Records", states: "The facility shall be operated and maintained in accordance with aporoved written procedures."
Requirements of the Operations Manual Chapter 2.1, authorizations and approvals, states; (Paragraph 1) the Reactor Director mest authorize in writing each day's operations as well as any maintenance, alterations, or tests.
(Paragraph 3) Alterations and nonroutine maintenance or tests

must be approved in advance by the ROC. The Reactor Director will prepare Part A of a Maintenance Request Forn and obtain the necessary approval. The person responsible for carrying out the alteration (change), maintenance, or test completes and signs part B.
---In part C, the Reactor Director inspects the nonroutine maintenance or alteration, and the records and approves their completion.
Contrary to the above, on February 1,1980, a new adjustable snubber was made modified and installed on the safety rod, the licensee failed to:
(1) generate a procedure for this nonroutine maintenance alteration; (2) authorize in writing this naintenance; (3) to receive ROC approval; (4) prepare a maintenance request; and (5) inspect the alteration and the records and approve their completion.
This is a Severity Level IV violation (Supplement 1).
l Pursuant to the provisions of 10 CFR 2.201, the Unive sity of California, Santa Barbara is required to submit to this office within thirty days of the date of this flotice, a written statement or explanation in reply, including:
(1) the corrective steps l
which have been taken and the results achieved; (2) corrective l
steps which will be taken to avoid further items of noncompliance; l
and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as anended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
