FINAL REPORT WASHINGTON PUBLIC POWER SUPPLY SYSTEM DOCKET NO. 50-397 LICENSE N0. CPPR-93 10CFR50.55(e) REPORTABLE CONDITION NO. 89 SPECIAL D0 ORS AND HATCHES POTENTIAL PROBLEM Deficiencies have been identified relative to Quality Class I special doors and hatches, which involve materials, fabrication, testing, and documentation. These doors include watertight doors in the basement of the reactor building, airtight doors and hatches in the reactor and ce, trol buildings, and blast doors in the reactor and control buildings.
Failure of the doors and hatches to perform their intended function could adversely affect the safe shutdown and the control of radioactive releases during a design basis accident condition.
A question existed as to whether the doors and hatches would have failed in service if the indicated conditions had gone undetected.
APPROACH TO RESOLUTION OF THE PROBLEM Evaluate all deficiencies for a particular door, hatch or sets of doors to determine if the deficiencies would have led to inservice failures; the evaluation to include non-destructive examination of the hardware in conjunction with an engineering review of the vendors design calculations.
Establish a task force of Supply System, Architect-Engineer, Construction Management, and Contractor representatives to review all software deficiencies.
Issue direction for correction of all deficiencies as required to meet design criteria.
STATUS OF PROPOSED RESOLUTION A task force was formed to identify deficiencies, provide technical resolution, and close out deficiencies associated with various types of special doors and hatches in an expeditious manner.
This work has been accomplished.
FINAL EVALUATION All of the deficiencies have been reviewed, evaluated, and resolved with the exception of those related to the 8H watertight doors (422' elevation, Reactor Building). The welding deficiencies on the 8H doors deviated significantly from the design specification / drawings and would have required a comprehensive engineering evaluation to determine if these doors, in this as-built condition, would perform their intended function.
It was concluded that this extensive engineering evaluation was not economically warranted.
The 8H doors have been returned to Rayproof, Inc. for extensive repairs which will bring them into conformance with design / specification requirements.

SUMMARY
Special doors / hatches 81, 8J, 8K, 8L, 8M, and 8N have been reviewed and evaluated for potential reportability under 10CFR50.55(e) criteria and have been determined to be nonreportable. All deficiencies noted have been resolved by normal site nonconformance procedures.
Special doors 8H have been reviewed and evaluated for reportability under 10CFR50.55(e) had have been determined to be reportable.
These doors are scheduled for repair which will bring the doors into conformance with design criteria.
