Identification of Occurrence: Scram dump tank level switch failure to trip.

5 Conditions Prior to Cccurrence: Plant shutdown to repair turbine stage drain leak.

Description of occurrence: Scram dump tant switches associated with the reactor protection system were being functionally tested l

following a report of pc7 reset operation of Switch RDOS-B.

Switch RDOS-D failed to trip properly. The switch was repaired j

and adjusted and retested for proper operation.

Analysis of Occu.rence: The failure ocouseed during switch esting with the pisnt is shutcovn condition. Previous testing on W

9 Corrective Action: The s' itch was repaired and tested'on June k, 1974. All four switches v1.

se tested satisfacterily prior to start-up. The vendor vill also oe contacted for any further recoc=endations.

10. Failure Data: The previous failures of this type of switch, Magnetrol 402-I, occurred on May 21, 1964 and Septe=ber 3, 1964.

Those failures were attributed to binding action of the flexible leads on the evitch housing cover. In September of 196k, the l

covers of all protection system svitches of this type in the l

plant vere replaced with larger covers to preclude that type of binding problem.

Switcl ?D08-D failed to trip properly. The switch was repaired and au.asted and retested for proper operation. 7 Designation of Apparent Cause-Possible component deterioration or increased friction in the float or svitch mechanism and/or binding action in the movable switch leads. 8. Analysis of Occurrence: The failure occurred durics switch testing with the plant in shutdown condition. Previous testing on about May 2,197h resulted in proper tripping perfomance cf all four durp tank high-level switches. Since the three other switches vera operable, the scram function vould have perfomed properly if required, due to the one out of two logic for each of the two protection channels. 9 Corrective Action: The switch was repaired and tested 'on June 4, 1974. All four svi cches vill be tested satisfactorily prior to start-up. The ver. dor vill also be contacted for any further recc==endations. l l 10.
