7 Description of Occurrence: The weekly Reactor Protection Icgic System Test is normally scheduled to be run each Tuesday. Due to j
oversight, the test was not conducted until Sunday, March 31, 1974.

Designation of Cause: The test form which had been used for many years ht.d been set aside for format revision and reappzcTal and was not Wnistratively authorized in timely fashion.
9 Analysis of Occurrence: The test is not a Technical Specifications

test, but has been in use since 1963 as a result of a nontail-safe j
logie cystem failure at that tira. It was conducted daily zor some period of time and revised for weekly testing several years ago.
It has always been performed without equipment failure cince 1963 1
and, based on the record, the failure to conduct the test in a timely fashion is not deemed to have inposed an undue nuclear safety hazard j
at the facility.
