Dear Mr. Heishman:
Our letter (Reference 1) responded to a notice of apparent non-compliance with NRC requirements as a result of an earlier inspection.
As a result of further inquiries concerning the circumstances of the activities which were noted by the Resident Inspectors, the maintenance personnel who were assigned to perform the work assoc-lated with a Special Work Permit have declared that although they periodically read their dosimeters during the work day, they are I
now unsure of the exact times that the readings were recorded.
This uncertainty was prompted by a review of the Machine Shop, Control Room, and job site copies of the SWP which were located in Health and Safety Department files and reviewed after the prepar-ation of our response.
The data entered on these forms was not consistent with the earlier statements which had been made from memory to the best of their knowledge.
Our letter (Reference 1) was prepared on the basis that the state-ments of maintenance personnel represented the most accurate source of information available since the copies of the SWP could not be immediately located. 8105 21 u jlt/JT" AP'R 2 0 W

R. F. Heishman, Chief LAC-7480 Reactor Operations and Nuclear April 16, 1981 Support Branch The original copy of the SWP which is retained as the master file copy and contains transcribed data had been reviewed by the depart-ment supervisor.
Since he had noted that data had been entered on the form it appeared to be in order and no further questions arose regarding completeness.
The confusion over the completeness of dosimetry information on SWP's will be alleviated by the use of color-coded copies with appropriate instructions to ensure that, where required, dosimetry readings are logged at the immediate job site rather than at admin-istrative offices.
When surface contamination from the job site precludes retention of the local records, appropriate transcription to a file copy will be performed.
The above practice is to be instituted upon printing of useable forms which are estimated to be available by May 15, 1981.
In-the meantime, clcser control over non-color-coded job copies of SWP's will be exercised.
