~P With the unit operating at 37% power, annameistor " Torus Vent Valves Open" alarmed, alerting the aperator to a possible loss of primary caetainment

isolation capability.

Investiaation showed that a needle valve in the air supply to the boot seal of the inmer isolation valve AD-2511 was closed.

and its boot seal is believed to have been deflated.

n e outboard isolation valw A0-2512 was blocked for construction modification to the air supply system to the valve.

In this condition, the boot seal on that butterfly valve was def1sted and the valve was inoperable.

Apparent Cause of Occurrence:-

Due to the construction work t= Mag place on AO-2512 in close proximity to 2 -2511, the needle valve is the supply to the boot seal was inadvertently brushed against and closed.

Samil fitting leaks between the needle valve-and the boot seal caused the volume to depressurize sufficisatly to cause i

-- Although the boot seals are believed to have been def1sted on both isolation v=1ves, the butterfly portice of the valves were closed. Thus a restricted j
