November 2h. 1980 Identification of Occurrence:
i With the Unit operating at 995 power while performing HPCI Surveillance Test 6.5 MD-3-23-1k EPCI Steam to Turbine Valve kept driving in the close direction causing the valve fuses to blev.
Conditions Prior to Occurrence:
Unit 3 operating at essentially full power; Unit 2 operating at T9 25 power.
Apoarent cause of Occurrence:
The torque switch which opens the feed to the close circuit of the valve fathd to open, which caused the motor to keep driv-ing the valve in the clamed direction until the motor fuses blev.
Analysis of Occurrence:
The Unit 3 HPCI system was inoperable for approvintely one half hour until tame fuses were replaced.
During this time all other Unit 3 BCCS systems were sparshle.
Due to the short period of inoperability safety significance is

Currective Action:

[ EThe EPCI valve h 3-23-lk elosing torque switch was inspected, readjusted and n-N valve was stroked satisfactorily.
