When a purchace requisition in roccived by the staff l

Assistant for Inventory Control-Nuclear (S.A.I.C.), the following procedure has been implemented to avoid-a future recurrence of the i

referenced infraction.

l If the recommended vendor is not on the Approved Bidders List and is not the original equipment manufacturer, the S.A.I.C.

contacts the originating station and inf,orms them that the material cannot be purchased from this vendor.

A rc., quest will then be sub-mitted by the originating station to evaluate the vendor and place them on the Approved Bidders List por Q.P. 7-51.

- and there is not sufficient time availabic to cvaluate and approve a vendor's Q.A. Manual, the procedure found in Q.P. 4-51 Attach-ment A, Section 3.0, paragraph 5, is implemented.

The' attention given thin infraction coupled with the procedure revicw by the parties concerned should prevent recurrence.

The apparent' incorrect _ operation of the LPCI loop select logic system was noted on September 29, 1975 following a reactor scram.

The respon'aible operating.cngineer was cognizant 'of the situation and= initiated activitics to dctormine the specific condi-tions and corrcctive action.

The action required by DAP 2-12 was to initiato a deviation repor:t and the required review to deterninc appropriate operating limits and corrective measurcs.

The deviation report was initiated on October 9, 1975, after the reactor had bocn returried to cervice.

In retroupact, it is our judgment that had the;cauce of the inccrrect functioning.of the loop select logic system been identified prior to returning the reactor to servico, the corrective' action would have.bcon required

' prior to return to cervicc. -Due to tho'dclay in preparing the do-viation report, the cause of the incorrect functioning van not known at the time of startup.

This matter hac been-thoroughly re-viewed by the responsible operating engineer and the importance of following procedures in these and similar circumstances ic* clear.
