NOTICE OF VIOLATION I

'2 Commocwealth Edison Company Docket Nos. 50-10, 50-237, and 50-249.

Based on the inspections conducted on November 10, 11, 15-18, and 22-23, 1977, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Items 1 through 4 are infractions, and item 5 is a deficiency.

1.

Contrary to 10 CFR 50, Appendir B, Criterion V, and the lic,ensee's Quality Assurance Frogram, maintenance work was performed on several Unit 2 control rod drives without tagging this equipment out of service as requested by the i

refuel foreman and as required in QP3-52. The failure to tag these control rod drive units out of service resulted in the expulsion of blade guides from the reactor core.

The licensee did take the CRD hydraulic control system

, charging line out of service, bewever, this did not pre-vent inadvertant blade guide expulsion on the individual CRD's lef t in the withdrawn position, when the hydraulic systee was returned to service.

2.

Contrary to Unit 2 Technical Specifications section 6.2. A, the licensee failed to follow procedure DNP 255 which 4

required the 2/3 diesel generator cooling water pump out s

i of service prior to working on LPCI valve 1501-3A. This resulted in 3 feet of river water flooding the east LPCI Toom.

Contrary to Unit 2 Technical Specifications section 6.2.A, i

the licensee failed to adhere to Dresden Administrative

]

Frocedure DAP3-5, in that system valve lineup on the 2A recirculation pump was not identified on the equipment outage i

checklist, resulting in the November 2, 1977 overpressuriza-tion of the 2A recirculation pump bowl and associated piping to 1435 psig.

The use of this inadequate procedure resulted in advertently lineup upon completion of the hydro is not specified.

Contrary to 10 CFR 50, Appendix B, Criterion V, and the licensee's Quality Assurance program, the licensee failed to maintain control copies of procedures DOP 4400-4, 5, 6, 7 and 8 in the Unit 2/3 control room, as required by DAP 9-3.
