Infraction:

Contrary to 10 CFR 71.3, Type B quantitics of byproduct material were delivered to a carrier for transport on sixty-six occasions during January, February, and March, 1977, without authorization by the Commission.

Corrective Action Taken and the Results Achieved:

Prior to June 22, 1977, periedic shipments of 55-gallon DOT specification 7A drums containing greater than 3 curies of Transport Group III L.S.A. materials packaged in shipping casks were routincly made from Dresden.

Station parsonnel incorrectly assumed these shipping casks, supplied by a vendor, had all required NRC certifications.

The vendor had applied for NRC certificates of compliance for these casks and they were issued by the NRC cn July 22, 1977, and August 12, 1977.

Corrective Action to bo Tchen to Avoid Further Noncompliance and Date of Full Cemoli:ince:

Since June 22, 1977, no drums containing greater than 3 curies of solidified waste have been shipped.

The shipping casks which will be used in the future to ship drums containing greater than 3 curics now have NRC certificates of compliance.

Full compliance has been achieved at this time.

Deficiency:

contrary.to Technical Specification 6.6.C.1, gaseous radionuclides released from Unit 2/3 chimncy and the Unit 2/3 reactor building ventilation stack have not been adagi ately quan-tified and reported.

l Corrective Action Taken and the Results Achieved:

a)

Chimney Releases After the installation of the augmented off gas treatment systems on Unit 2 and 3 in July, 1974, and prior to July 6, 1977, releases from the Unit 2/3 chimney had been quantified by summing the offluent releases from the Unit 2 and Unit 3 charcoal trains.

This technique did not allow for quantification of noble gas activity released to the chimney from the gland seal steam and ventilation air.

With the installation of the augmented off gas treatment systems which reduced off gas system nobic gas releases by a factor of approximately 1000, these sources remained small but a larger percentage of the total.

In order to adequately quantify and report total chimney noble gas releases, beginning July 6, 1977, noble gas sampling from the Unit 2/3 chimney was initiated on a Monday through Friday basis.

These sampics indicate a total chimney noble gas relonce rate of between 700 and 2700 uCi/sec.

b)

Roactor Building Vent Stack Releases Nobic gas releases from the Unit 2/3 reactor building ventilation stack were assumed to be extremely low because continuous stack monitoring equipment had not indicated the presence of any significant noble gas releases.

In order to verify this fact and adequately quantify and report reactor ventilation stack noble gas releases, beginning July 9, 1977, samples from the Unit 2 and Unit 3 reactor building ventilatica systems have been analyzed twice per week.

Results have generally been less than 10 uCi/sec.

To avoid further noncompliance, samples from the Unit 2/3 chimney will be collected and analyzed 7 days por weck.

samplos taken once per week frem the Unit 2 and Unit 3 reactor building ventilation exhaust ducts.

This sampling frequency will continue until the noble gas monitoring system on the Unit 2/3 reactor building ventilation stack is operational.

Lastly, adequate reporting of these releases will bc

made by appropriate changes to the semi-annual report, starting with the July to December, 1977 Report.

Date of Full Complianec:

Full compliance is pending calibration of the sodiun iodido well counter and procedure development and implementation.

This is expected to be completed by September 30, 1977.
