SUBJECT: NRC INSPECTION REPORT NO. 50-62/93-01
This refers to the inspection conducted by P. T. Burnett of-this office on
May 3, 1993.
The inspection, the first of two, included a review of activities
'
authorized for your University of Virginia Reactor facility. At the conclusion
of the inspection, the findings were discussed with those members of year staff
identified in the report.
Areas examined during the inspection are identified in the report. Within'these
areas, the inspection consisted of selective examinations of procedures and
representative records, interviews with personnel, and observation of activities
'
in progress.
The issues identified during this inspection will be further reviewed during a
subsequent inspection tentatively scheduled for early June. Accordingly, no
Notice of Violation is being issued with this report. The issues involve
improperly controlled maintenance activity, which lead to the defeat of five
required reactor safety features used for the prompt shutdown of the reactor.
The Technical Specifications for the reactor require that these features be
fully functional whenever the reactor is operating as it was on the day of the
event.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this
letter and its enclosure will be placed in the NRC Public Document Room.
