-SEQUOYAH RESTART ISSUES This refers to the meeting conducted at our request at the Sequoyah Nuclear Power Plant on April 5, 1993. The purpose of the meeting was for you to present to the NRC Restart Panel the status of your actions to identify items and/or programs to be corrected prior to restart of the Sequoyah Units.

MEETING

SUMMARY

The initial meeting of the Sequoyah Restart Review Panel was called to order by the Chairman Mr. A.F. Gibson. Mr. Gibson then presented an outline for the meeting. The purpose of the meeting was to develop a draft restart Action Plan, a draft Restart Checklist, and draft inspection charters.

The Panel then commenced a review of the checklists contained in NRC Manual Chapter (MC) 0350. The decision was made to utilize those items from the generic checklist of MC 0350 and create a site specific checklist.

An overview of the Augmented Inspection (AIT) team findings was presented by Mr. J. J. Blake, the AIT Team Leader. The root causes of the failure of the erosion / corrosion program were identified to be failure of management to clearly assign responsibilities and provide appropriate oversight and direction for monitoring and maintaining balance of plant piping. Mr. Blake indicated that TVA had missed several opportunities to identify the degraded piping and the CHECKMATE program which was intended to predict piping degradation was not effective because of implementation deficiencies.

The Panel then established a generic inspection outline for each inspection to be conducted prior restart.

The generic outline consisted of an inspection charter, a mission statement, proposed dates for the inspections and the inspection leader.

The Panel then attended a meeting with TVA to ascertain the actions that TVA thought would be necessary to complete prior to restart.

The agenda of this meeting is attached as well as TVA's handout concerning the development of restart items.
