Modification No. 4 to Task Order No. 01 Under Contract No. NRC-33-92-203 The purpose of this modification is to: 1) add travel to the NRC Regional Offices in accordance with the attached statement of work, 2) increase the contract ceiling amount, and 3) delete Alan Dolleck as Task Manager for this task order and designate John Burton as the new Task Manager.
Travel shall-be performed in accordance with the contractor's revised cost proposal dated February 1,1993 which is hereby incorporated into the task order by reference.
The ceiling amount of this task order is increased by $12,654.46 from
$239,383.82 to $252,038.28. The amount of $242,344.50 represents the reimbursable cost and the amount of 59,693.78 represents.the fixed fee.
Your contacts for technical matters during the course of this task order are revised as follows-Technical Matters: John Burton Project Officer and Task Manager (301) 492-4836 All other terms and conditions of the subject task order remain unchanged.
Please indicate your acceptance of this Modification No. 4 to Task Order No 1 by having an official, authorized to bind your organ'zation, execute three copies of this document in the space provided and return two copies to the Contract Administrator. You should retain the' third. copy for your recordt.
