MILLSTONE COMBINED INSPECTION 50-245/93-10; 50-336/93-06;

50-423/93-07

This refers to the safety inspection conducted by Mr. P. Swetland and others of this office on

March 3,1993 through April 3,1993, at Millstone Station in Waterford, Connecticut. The

,

preliminary findings were discussed with Mr. S. Scace, the Unit Directors, and others of

,

your staff at the conclusion of the inspection. Areas examined during the inspection are

i

l

described in the enclosed report. Within these areas, the inspection focused on issues

l

important to public health and safety, and consisted of performance observations of ongoing

i

activities, independent verification of safety system status and design configuration,

interviews with personnel, and review of quality records.

The inspectors identified that certain procedural requirements of the Unit 2 license were not

adequately implemented during this inspection period. These included failure to follow test

procedures which resulted in inadvertent reactor protection system actuations, and the late

reporting of such actuations to the NRC. We also found that your corrective action for

electrical contact failures at Unit I was not effective in precluding recurrence of these

failures. Three violations of NRC requirements concerning these problems are cited in the

enclosed Notice of Violation.

You are required to respond to the enclosed Notice of Violation and should follow the

,

instructions in the enclosed Notice when preparing your response. In accordance with

10 CFR 2.790 of the NRC's " Rules of Practice," a copy this letter and the enclosed Notice

will be placed in the NRC Public Document Room. The responses directed therein are not

subject to the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, Public Law No. 96.511.

During this inspection, we also evaluated your self-identification and correction of problems

related to an open reactor building blow-out panel at Unit 1, unauthorized conduct of

surveillance tests at Unit 2, and inadequate surveillance test implementation at Unit 3. We

have determined that your corrective actions for these issues were acceptable and the specific

problems posed minimal safety consequences. Therefore, in accordance with Section VII.B

of the Enforcement Policy, enforcement discretion was exercised and violations were not

cited for these cases.

Our inspection also involved a detailed review of the activities of groups that contributed to

the overall self-assessment function at Millstone. Our review, documented in Section 5.4 of

the enclosed report, found generally mixed performance and identified a number of

significant opportunities for improvement. I believe this area of the report warrants your

close attention and I would be interested in hearing from you if you either have questions or

dispute the findings.
