SUBJECT: NRC INSPECTION REPORT 50-445/93-10; 50-446/93-10 (NOTICE OF
VIOLATION)
This refers to the inspection conducted by Mr. M. Runyan, and the inspectors
identified in the enclosed report, of this office on March 15-19 and on
April 7,1993, during which a supplemental exit meeting was conducted. The
inspection included a review of activities authorized for your Comanche Peak
Steam Electric Station, Units 1 and 2, facility. .At the conclusion of the
inspection, the findings were discussed with you and those members of your
staff identified in the enclosed report.
The inspection examined the program implementing your comitments to Generic Letter 89-10, " Safety-Related Motor-Operated Valve Testing and Surveillance."
Within this area, the inspection consisted of selective examination of
representative records and interviews with personnel.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). Violation A is of concern because the failure to evaluate
the recomendations of vendor technical documents could affect the operability
of safety-related components.
Two unresolved items were iden'.ified during the inspection. One involved an
incorrect calculation of torque limits for a valve test and the subsequent
failure to independently verify the calculation. The other involved an open
stroke dynamic diagnostic test that appeared to have been performed without
the valve being in a fully closed position. These issues are discussed in
Section 1.2 of the enclosed inspection report. At the exit meeting, you
comitted to provide additional information on these two issues by April 2,
1993.
Based on the additional information you provided on March 30, 1993, and the
supplemental information you supplied during a telephone conversation on
April 5, 1993, between Mr. Runyan and Messrs. O. Bhatty and R. Withrow, the

failure to follow procedures and to provide an independent verification of
motor-operated valve test input data has been identified as Violation B as
t
specified in the enclosed Notice. As a result of your accomplished and
committed corrective actions in response to the identification of this issue,
i
you will not be required to respond to this violation.
The other unresolved item identified during the inspection, concerning
apparent nonconservative initial test conditions, has been reclassified based
i
on further review as an inspection followup item.
The inspection revealed that you are relying on a study conducted by Kalsi
Engineering of the structural thrust capability of certain Limitorque
u.'uators, as discussed in Section 1.6 of the enclosed inspection report. The
r
aff's position regarding the use of this study to increase actuator thrust
ratings will be provided separately by the staff.
You are required to respond to this letter and should follow the instructions
,
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
!
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC's Public Document
Room.
The response directed by this letter to the enclosed Notice is not subject to
the clearance procedures of the Office of Management and Budget, as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
