Office of Nuclear Reactor Regulation Items of Interest Week Ending April 16, 1993 l Diablo Canyon - Eaale 21 Reactor Trio System (RTS)/Anticioated Transient Eithout Scram (ATWS) Mitiaatina System Diversity During the staff's Eagle 21 RTS retrofit audit at Westinghouse in Pittsburgh on April 5 & 6, and PG&E on April 7, 8, & 9, the staff learned of an impending concern regarding the level of diversity between the Diablo Canyon ATWS mitigating system and the proposed Eagle 21 RTS. Both systems are digital and are manufactured by Westinghouse using similar Intel processors, the same programming language, the same compiler and identical major hardware components such as multiplexers and isolation amplifiers. Westinghouse is providing additional justification for the degree of diversity between the RTS and the ATWS system. A meeting at White Flint between NRC, Westinghouse and PG&E will follow the submittal of this justification. The requirement for diversity is stated in the ATWS Rule,10 CFR 50.62. NRC Workshop on GL 89-10 Implementation On April 8,1993, a workshop on GL 89-10, "Safei.y-Related Motor-0perated Valve Testing and Surveillance," was held among the NRR and Region management and senior inspectors in the Region IV office. The purpose of this meeting was to discuss and resolve technical and policy concerns related to the continuing implementation of this program. Significant accomplishments at this meeting include proposing the use of valve grouping under certain conditSns as an option for establishing design-basis valve setup; resolution of seeral technical positions related to inspection guidance; and establishing direction and targets for further generic communications. These inciude respwse to questions and answers from the GL 89-10 public workshop held in k'oenix, Arizona, on February 24, 1993, and information to the industry on NRC expectations regarding confirmation of MOV capability and scheduling. Consensus was reached among NRR and the Regions on all significant issues addressed. Secuoyah. Units 1 and 2 On April 12, 1993, the NRC staff met at the NRC Headquarters in Rockville, Maryland, with representatives of the Tennessee Valley Authority (TVA) senior management to address TVA's assessment and corrective actions related to restart of the Sequoyah Nuclear Plant. The topics discussed included recent problems, plant status, development of plant restart criteria, performance and evaluation techniques, program enhancemer ts, status cf balance of plant work and backlogs, restart assessment and scht dule, and TVA Corporate support. Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending April 16, 1993 Russian Technical Exoerts Continue Tour of U.S. Facilities A delegation of 24 Russian technical experts in material control and accounting (MC&A) and physical protection continued their tour of U.S. nuclear facilities and discussions with Nuclear Regulatory Commission and Department of Energy (D0E) personnel. The meetings, which began with a technical exchange briefing in Alexandria, Virginia, on March 29, 1993; followed by a visit to the North Anna, Virginia, reactor on March 30, 1993; continued with a visit to the General Electric Nuclear fuel and Components manufacturing plant in Wilmington, North Carolina, on March 31 and April 1, 1993. The tour and presentations by facility staff and NRC allowed for the explanation of NRC inspection procedures for internal controls and measurement systems, NRC + requirements regarding the independence of MC&A functions from production responsibilities, as well as implementation of NRC's MC&A requirements. The remaining tour for the delegation included three DOE facilities. The April 2, 1993, site visit to Oak Ridge, Tennessee, was for the purpose of reviewing aspects of the Nuclear Materials Management and Safeguards System (NMMSS) and facility tours. Concurrent visits to Sandia National Laboratories and Los Alamos National Laboratory on April 5-6, 1993, were to allow for U.S. and Russian presentations in physical security and MC&A, as well as facility tours and demonstrations of U.S. technology. The trip concluded at Sandia on April 7,1993, with day-long discussions and collaboration on a summary statement of the technical information exchange. The statement includes possible areas of cooperation between the two countries based on the text of the agreement finalized in Moscow on March 26, 1993, and the next steps for the program of cooperation. Material Control and Accountino Insoection Division of Fuel Cycle Safety and Safeguards staff are conducting a Material Control and Accounting inspection the week of April 19-23, 1993 at Combustion Engineering, Hematite, Missouri. Areas that will be inspected include system j management and assessment, internal controls, measurement systems and controls, records and reports, and follow-up on unresolved items. i Public Meetino on the Desian Basis Threat A public meeting is scheduled for May 10, 1993, at the Crowne Plaza Holiday Inn, Rockville, Maryland, to obtain information from members of the public, licensees, and other interested parties relevant to the Nuclear Regulatory Commission's review of the design basis threat for radiological sabotage. The NRC is reevaluating the threat statement, which is contained in 73.l(a)(1) of NRC's. regulation on Physical Protection of Plants and Materials (10 CFR Part 73), in light of public concern and the recent incidents of a vehicle intrusion at the Three Mile Island power reactor and a vehicle bomb explosion at the World Trade Center. i APRIL 16, 1993 ENCLOSURE B

2 s Interested parties are expected to bring forth information regarding such topics as adversary size, capabilities, and attributes, adversary use of a vehicle to gain access to the protected and vital area, and as a weapon or bomb to commit radiological sabotage. U.S. Department of Aoriculture On April 13, 1993, representatives from the materials licensing and inspection staff in Region I and the Medical, Academic and Commercial Use Safety Branch, Office of Nuclear Material Safety and Safeguards, met with managers from several agencies within the U.S. Department of Agriculture (USDA). The meeting was held at the request of USDA to discuss the inspection findings which resulted in the issuance of a Confirmatory Order requiring the perfor-mance of an independent audit of USDA's Radiation Safety program. The Nuclear Regulatory Commission's expectations were presented and a discussion ensued regarding the practical aspects of implementing more effective management oversight of radiation safety at the more than 300 individual facilities using byproduct material under the USDA's license. USDA managers identified a major concern that communications between users of byproduct material and the Radiation Safety Staff often bypassed line management within the various agencies. USDA managers expressed their strong support of initiatives to improve the effectiveness of their radiation safety program. Audit of the Yucca Mountain Site Characterization Pro.iect Office On April 5-9,.1993, quality assurance (QA) staff of the Division of High-Level Waste Management (HLWM) observed an internal QA audit of the Yucca Mountain Site Characterization Project Office (YMPO) by the Yucca Mountain Quality Assurance Division (YMQAD). This audit evaluated the Office of Civilian Radioactive Waste Management (OCRWM) QA program as implemented by YMP0 to determine whether it meets the OCRWM requirements. Eight of the applicable QA programmatic elements were evaluated by the audit team. Neither technical areas nor YMQAD were evaluated by the audit team. The State of Nevada did not participate in this audit. The HLWM observers determined that the audit was effective and agreed with the audit team that YMP0 implementation of the OCRWM QA program was effective. As a result of the audit, four preliminary Corrective Action Requests (CARS) were issued by the audit team: three against the YMP0 and one against the Management and Operations Contractor (M&O). The adverse conditions identified in the CARS are not significant in terms of the overall OCRWM QA program as implemented by YMP0 and the M&G. Briefina to U.S. Environmental Protection Aaency On April 13, 1993, staff of the Division of High-Level Waste Management (DHLWM) briefed staff of the U.S. Environmental Protection Agency (EPA), Criterion Standards Division and Waste Isolation Pilot Project (WIPP) Work Group responsible for the certification of compliance of the WIPP with EPA's standards for transuranic waste. The briefing was held at the request of the EPA staff to familiarize them with the types of prelicensing activities conducted by the DHLWM. 3 o an overview of the DHLWM organization, including the responsibilities and activities of the Nuclear Regulatory Commission On-Site Licensing Represen-tatives. Nuclear Regulatory Commission representatives also discussed the licensing process and requirements, development of review plans, how the staff r conducts prelicensing reviews of Department of Energy (DOE) submittals, and the various requirements for NRC/D0E public meetings and dissemination of information relevant to the high-level waste project. Status of Drillina and Blastina of Exploratory Studies Facility North Portal Blasting of the first pilot hole of the Yucca Mountain Exploratory Facility (ESF) North Portal starter tunnel took place at 4:15 p.m., Tuesday, April 13, 1993. The drawings specifications, work procedures and quality assurance (QA) checklist associated with the drilling and blasting operations were reviewed by the Nuclear Regulatory Commission On-Site Licensing Representatives (ors) and were discussed with technical and QA representatives of the on-site Department of Energy contractor, Reynolds Electrical and Engineering Company, Inc. (REECo). Specific process " hold points" are identified on the drilling and blasting specification checklist for the REECo QA staff to inspect and a sign off on prior to proceeding. Office of Nuclear Regulatory Research i Items of Interest Week Ending April 16, 1993 Enhanced Particioatory Rulemakina Workshoo in Philadelohia The fifth in the series of workshops in the enhanced participatory rulemaking on radiological criteria for decommissioning was held in King of Prussia, Pennsylvania on April 13 and 14, 1993. A total of 22 individuals (excluding NRC and EPA staff) participated in the discussions around the table. The individuals represented State government, local government, fuel cycle licensees, decommissioning contractors, environmental groups, nuclear utilities, and professional societies. A representative of Indian tribes was not in attendance due to the unfortunate cancellations from the National Congress of American Indians and the Shoshone-Bannock Tribe. The discussions were open and active, with excellent participation by all of the attendees. Of particular note was the extended discussion of generic versus site specific considerations, where a distinction was made between the need for some generic criteria while maintaining the ability to implement those criteria taking into account site specific considerations. A second major theme throughout the discussions was the need for continued public involvement in all aspects of decommissioning, both in the development of the standards and in their implementation at a particular site. Observers from a number of licensees and various citizen groups were in attendance. Opportunities were provided for these observers to provide their thoughts on each topic discussed by the participants. On the evening of April 12, 1993, the NRC and EPA staff held an informal public session from 7:00 to 10:00 pm for members of the public who would not otherwise be able to attend the workshop sessions due to work or other commitments. This session was also very productive, with many comments on each of the areas in which comments have been solicited. Release of MELCOR 1.8 J A period of extensive modifications and updating of the NRC's severe accident analysis code MELCOR has been completed with the release of MELCOR version 1.8.2 on April 7, 1993. The modifications and improvements accomplish many of the goals set in response to the FY91 MELCOR Peer Review (ref.: LA-12240). The MELCOR code models the entire progression of severe accidents in light water nuclear power plants including reactor coolant system and containment t thermal and hydraulic response, core heatup and degradation, fuel and core relocation, and fission product release and transport for both BWR's and PWR's. The recent improvements include modeling of direct containment heating (DCH) following high pressure melt ejection, modeling of containment ice condensers, modeling of reactor vessel lower plenum debris behavior and bottom i head heatup and failure, modeling of radial spreading of molten and particulate debris, modeling high temperature eutectic reactions and dissolution of solids in molten mixtures, and many changes to correct i operational difficulties of varying degrees of severity. The MELCOR peer review involved an expert review panel of eight people who performed an in-depth evaluation of the adequacy of the code and its APRIL 16, 1993 ENCLOSURE C F

F 2 e documentation for the NRC. After more than a year of work, half a dozen full committee meetings, utilization of more than 125 reference documents, and thorough analyses from the top down and from the bottom up, the group presented major findings and recommendations in five areas including needs for models missing from the code, improvements to models, improvements to code numerics, needs for additional assessment, and recommendations on documentation, in a 250 page summary report. Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending April 16, 1993 Diaanostic Evaluation and Incident Investication Branch (DEllB) The South Texas Project (STP) Diagnostic Evaluation Team (DET) is finishing the first of two weeks of in-office evaluations prior to returning to STP the week of April 26 for the final on-site evaluation period. Incident Response Branch (IRB) Director, D0A, and IRB staff members participated in a meeting of the Catastrophic Disaster Response Group on April 12 to discuss information related to the Federal Response Program (FRP) revision project. IRB staff participated in three meetings in Omaha, Nebraska this week to discuss preparations for the Fort Calhoun exercise to be held in late June, 1993. The groups that met included the Planning Committee, the Scenario Workgroup, and the control documentation workgroup. IRB staff attended a meeting of the Operations Work Group of the Federal Response Planning Task Force on April 15 to discuss revision to the FRP. Trends and Patterns Analysis Branch (TPAB) The Trends and Patterns Analysis Branch (TPAB) recently completed a number of technical reports documenting special studies as follows: 1. AEOD/S93-01, " Review of Auxiliary Feedwater System Reliability," issued April 2. This report documents the results of AE0D's screening analysis of operational experience with PWR auxiliary feedwater systems. The study provides a generic perspective on AFW reliability. 2. AEOD/593-02, " Operating Experience feedback - Reliability of Safety-Related Steam Turbine-Driven Pumps," which was transmitted for Peer Review on April 13. This report documents AE0D's study of operational experience and safety implications associated with failures and degradations of turbine-driven standby pumps at U.S. LWRs. The study provides data on failures of standby turbine-driven pumps in order to identify failure mechanisms and corrective actions, an estimate of the reliability of turbine-driven pumps, and data for use in reliability analyses. 3. AEOD/S93-03, " Operating Experience Feedback - Service Water System failures and Degradations," issued April 5. This report documents AE00's review and analysis of operational experience data for service water systems over the period 1987 through 1991. These data will be used to establish an industry-wide baseline of performance related to the concerns addressed in Generic Letter (GL) 89-13, and to develop a ) framework for evaluating the effectiveness of implementing the staff's Task Action Plan for resolving the problems with service water that have continued since the release of GL 89-13. i 4. AEOD/S93-05, " Operational Data Analysis of Shutdown and Low Power i Licensee Event Reports," issued. April 1. This report presents AE0D's analysis of operational experience during shutdown and low power operations over the period January 1985 through June 1992. The report also describes the approach which AECD intends to use to monitor future industry performance in this area of operations. i TPAB also completed two special studies in response to Commission requests, i both of which were issued April 1. The first consisted of an evaluation of. the plateauing of the performance indicators which was performed in response i to the Commission's 10/8/92 SRM. The second consisted of an analysis of LERs i reporting problems which were discovered during a design basis review. Office of Administration Items of Interest Week Ending April 16, 1993 List of Anoroved Storace Casks: Additions A final rule entitled " List of Approved Storage Casks: Additions" was published in the Federal Register on April 7, 1993 (58 FR 17948). The final rule adds an additional storage cask to the list of approved storage casks in 10 CFR 72.214. The final rule allows holders of power reactor operating licenses to store spent fuel in this approved cask under a general license. The final rule becomes effective May 7,1993. OPM Security Directors' MeetinQ Raymond Brady, Director of Security and James Dunleavy, Chief, Personnel Security Branch, attended a Security Directors' meeting on April 14, 1993 hosted by the OPM Assistant Director for Federal Investigations. These semi-annual meetings are hosted by OPM to discuss the status of various initiatives and topics of interest to agencies who use OPM personnel security background investigations. Criminal History Proaram Under the requirements of 10 CFR Part 73, the NRC Division of Security (SEC) administers the Criminal History Program (i.e., processes fingerprint cards from licensees). During the period March 29 - April 9, 1993, SEC received 3,374 new fingerprint cards from and returned approximately 2,653 to l licensees. As of April 9, 1993, 2,395 cards were on-hand. Contract Awards On April 9, 1993 a contract was awarded to Systems Research and Application i Corporation for " Technical Assistance for Emergency Response." The contract provides a variety of services for NRC's Incident Response Program include on-call, 24-hour logistical and computer support. NRC has awarded a contract to Sumitomo Heavy Industries, Ltd., Tokyo, Japan, to design, fabricate and install instrumentation and equipment to modify the ROSA Large Scale Test Facility. This facility is located on the Japan Atomic Energy Research Institute. Tests will be conducted at the ROSA facility to obtain experimental data which will be used by HRC in its review of the Westinghouse AP600 design. The DCPM Contract Officer negotiated a favorable yen to dollar conversion ratio, resulting in a $280,000 reduction in the proposed cost of the contract. GSA Strateoic_Relreitt i Louis Fisher, Chief, Facilities Management Branch, Division of Contracts and Property Management, attended the General Services Administration's (GSA) Strategic Partnering Retreat held on April 14 and 15, 1993. l l Office of Information Resources Management Items of Interest Week Ending April 16, 1993 i Southwest Research Institute (SwRI) The Office of Information Resources Management staff continues to work with the Southwest Research Institute (SwRI) and the Office of Nuclear Material Safety and Safeguards (NMSS) staff to ensure all user requirements are met. The Message Handling System mail gateway between SwRI and White Flint was installed on April 9,1993. The mail gateway is operational and testing continues with SwRI, NMSS, and the Network Control Center (NCC) staff to ensure'its operability. In addition, the Internet protocol printer server was installed on April 9,1993. The printer server is operating and testing is continuing among SwRI, NMSS, and NCC. Computer Security Computer security plans were approved for the Freedom of Information Act (F0IA) Office Tracking System and FOIA Action Tracking System, Division of Freedom of Information and Publications Services, ADM; the Technical Training Center Exam Data Bank and the Institute of Nuclear Power Operations (INPO) Reports Data File, Office for Analysis and Evaluation of Operational Data; and the Foreign Documents Database, Office of Information Resources Management. ITS Trainino Lab All of the PCs in classroom 370 were upgraded from 386 to 486 class machines. AUTOS I Agency Upgrade of Technology for Office Systems (AUTOS) implementation is underway in the Office of Information Resources Management (IRM). The i Division of Computer and Telecommunications Services is on AUTOS, and installation and indoctrination is currently underway in the Division of Information Support Services starting with the Library Services Section. Office of Small and Disadvantaged Business Utilization and Civil Rights Items of Interest Week Ending April 16, 1993 Oak Ridge Associated Universities (0RAU), an NRC grantee, has made an offer and received an acceptance from Dr. Tom Whitney, Director of the Civil and Mechanical Engineering Department at South Carolina State University under a new NRC program. Dr. Whitney will work in the Division of Safeguards and Transportation, NMSS effective May 17, 1993 through the Summer 1993. Office of Enforcement Items of Interest Week Ending April 16, 1993 Significant Enforcement Action: A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $ 1,750 was issued on April 16, 1993 to Cameo Diagnostic Centre, Inc., Springfield, Massachusetts. The action was based on (1) the willful use of NRC licensed me+erial at a new location not authorized by the license, and (2) the willful failurs to inform the NRC of such use without regard to prior notice that such use at a new location was prohibited without a license amendment or renewal authorizing work at the new location. The willful violations have been categorized in the aggregate at Severity Level III. The base civil penalty was escalated 100% based on NRC identification of the violations; escalated 100% based on prior notice as noted above; and escalated 100% based on the duration of the violations. (EN 93-033) Civil Penalties Paid: Professional Services Industries, Inc., Lombard, Illinois paid the civil penalty in the amount of $7,500. The action was based on a violation that occurred at a field site in Wampum, Pennsylvania, where a radiographer and radiographer's assistant from the licensee's Pittsburgh, Pennsylvania office, did not maintain direct surveillance over the high radiation area created during the radiography operations. This violation resulted in two individuals entering the high radiation area and receiving unnecessary radiation exposure. The violation has been categorized at Severity level III. Tennessee Valley Authority (Sequoyah Units 1 and 2) paid the civil penalties in the amount of $225,000. The action was based on three Severity Level III problems associated with: (1) several mispositioned safety system throttle valves, (2) the failure to maintain the RWST temperature above technical specification limits, and (3) a loss of Reactor Coolant Pump seal injection during recovery from a dual unit trip on December 31, 1992. Coincident with extension of the driveway, the entrance to the OWFN garage is being widened and modified requiring relocation of the garage entrance identification key-card reader. l Two caulking teams continued closing seams on the east and south faces of j -TWFN. Installation of permanent lighting, ceiling grids in the garage on Level A, exterior doors, fire control room wiring, and various electrical and mechanical subsystems proceeded at a quickened pace. j Also during the week, the prime contractor conducted water leak tests. of 'the f exposed OWFN deck above the garage between the two buildings.. Leaks were identified and patched. ~ i Office of State Programs Items of Interest Week Ending April 16, 1993 i I Response to Conoressman Synar's Ouestions The Office of State Programs (OSP) staff worked on responses to the questions-submitted by Congressman Mike Synar in a March 25, 1993 letter to the Chairman.. Congressman Synar is conducting a review of the activities of the NRC in administering the Agreement State program under Section 274 of the i Atomic Energy Act. The Congressman requested files dating back, in some i cases, to 1980 on Agreement State reviews. The responses (5 sets) to these l questions were packaged in twenty boxes with a combined weight of approximately one thousand pounds. OSP support staff worked overtime, on weekends, and in some cases, throughout the night to complete the task of gathering, compiling and assembling the documentation. J Particular_ recognition goes to Debra Reyes, Gwen Davis, Kathaleen Kerr, Brenda Hill, Jesse Delgado (NRR) and Janice Hester (OC) for their remarkable effort i in coordinating and categorizing this massive undertaking. Southeast Compact to End Central Compact's Access to Barnwell The Southeast Compact Commission for Low-Level Radioactive Waste Management voted unanimously on April 14, 1993 to terminate its contract with the Central reaching its decision to deny access to the Barnwell low-level waste disposal . l Interstate Low-Level Radioactive Waste Commission effective July 1,1993. In i facility, the Commission acted on the recommendation of its Import Policy Committee which reviews out-of-region progress reports and develops recommendations for access continuation or termination. The Import Policy Committee had considered the Central Compact's status at a special meeting on l February 24, 1993. The Southeast Commission indicated that it would reconsider the denial if the Central Compact and Nebraska were able to provide a convincing plan of action prior to July 1. While voting to continue all i other contracts, the Southeast Commission expressed concern over the pace of development in the Midwest Interstate Low-level Radioactive Waste Compact, the Northeast Interstate Low-Level Radioactive Waste Compact and the State of New I York and indicated it will closely monitor progress in those entities. USDA Meetino The United States Department of Agriculture (USDA) invited representatives of the NRC, including John Glenn, Chief of the Medical, Academic and Commercial Use Safety Branch, DIMNS, and Elizabeth Ullrich, i Senior Health Physicist, Region I, to attend a meeting in Washington, DC on April 13, 1993, of the Administrators from all USDA Services which t possess and use licensed material under the broad scope and irradiator licenses issued by the NRC. John Glenn reviewed the rights and responsibilities of a broad scope license NRC expectations of such programs, and NRC enforcement policy. The meeting was hosted by the - Agricultural Research Service Office of the Administrator, to whom the Radiation Safety Officer and Staff report. The meeting also included-I discussions of the results of the 1992 inspections and resulting Enforcement Conference, projected corrective actions, and the plans for the upcoming audit of the USDA radiation safety program. The meeting was useful in ensuring all USDA services are aware of the required improvements in their radiation safety activities, 2. Vermont Yankee-On April 6, 1993, during a routine rod scram time surveillance test, the licensee identified that the average core wide scram insertion time for the notch 46 position exceeded the Technical Specification Limit of.358 seconds by 11 milliseconds. In addition, nine 2-by-2 rod arrays also i were in excess of Technical Specification limits. Based on'the licensee's evaluation of the safety significance of the increase in core average rod scram time, enforcement discretion was granted by the NRC l for 48 hours, to preclude an immediate plant shutdown. Subsequently, the plant was shutdown due to an unrelated plant leak in the feedwater system. The licensee formed task teams to review the root cause for the slow rod scram insertion times and to review the test methods and data evaluation i techniques. On April 10, the licensee reported that the slow scram insertion times were determined to be caused by the slow response of the i hydraulic control unit's scram solenoid pilot valves (SSPV). The licensee is replacing / rebuilding all SSPVs and will perform full core testing prior to startup. On April 12, the licensee reported that the i scram insertion time Technical Specification limit had been exceeded 'for ~ one 2-by-2 array during rod testing on October 15, 1992.. However,.the Technical Specification required action was-not understood and the= l appropriate actions were not taken. A Region I DRS Inspector was ( dispatched to the site on April 14, 1993, to assist the resident inspector review the information regarding this issue.

Region III Items of Interest Week Ending April 16, 1993 1. Allied-Sional. Inc.. - Paducah Gaseous Diffusion Plant On April 13, 1993, Chairman Selin, Congressman Glen Poshard, the Deputy Regional Administrator, Region III, and members of their staffs, toured Allied-Signal's Uranium Hexafluoride (UF6) Conversion Facility at Metropolis, Illinois and the Department Of Energy (DOE) Gaseous Diffusion Plant at Paducah, Kentucky. Chairman Selin held a press conference with six members from the local news media at the Allied Signal facility. After touring the Paducah facility, Chairman Selin held a meeting with DOE representatives, contractors and selected plant union officials. The impending transition to NRC regulation of the plant was discussed in this meeting. 2. Michiaan State University On April 15, 1993, an Enforcement Conference was conducted in the Region III office between representatives of Michigan State University and members of the Region III staff. The purpose of the conference was to discuss the findings from the special inspection conducted from March 10 through 23, 1993. The inspection was conducted to review an incident in which carbon-14 contamination was spread to unrestricted areas outside of the licensee's control. Nine apparent violations were identified during the inspection which included failure to properly control licensed material. The licensee did not contest any of the apparent violations during the conference. The results of the conference are under review and a final determination on enforcement action is pending. 3. Zion Nuclear Power Station On April 16, 1993, Commonwealth Edison Company management attended an enforcement conference held in the Region III office to discuss apparent violations involving the opening of an auxiliary building missile door to set up an external laundry facility. Licensee attendees included the Zion Site Vice president, the Zion Station Manager, and members of their staff. NRC attendees included the Deputy Regional Administrator, the Deputy Director, Division of Reactor Projects, and members of the Region III staff. 4. Braidwood Nuclear Power Station On April 16, 1993, Commonwealth Edison Company management attended an enforcement conference held in the Region III office to discuss an apparent violation involving the at-power operation of Unit 2 with the reactor head vents inoperable. Licensee attendees included the Braidwood Site Vice President, the Braidwood Station Manager, and members of their staffs. Unit 2 Status Comanche Peak Steam Electric Station, Unit 2 was synchronized successfully to the electrical grid for the first time at 8:03 p.m CDT on April 9, 1993. The unit is currently being held at 28 percent power while adjusting nuclear instrumentation and bringing steam generator water chemistry within limits. The next plant operating milestone is the 50 percent operating plateau which is scheduled to be reached on April 21, 1993. Significant NRC activities scheduled at this plateau include a regional assessment of the licensee's readiness to proceed above 50 percent power, and the observation of major tests such as remote plant shutdown. f 2.
