This letter reports. In accordance with the requirements of Technical Specifications for Facility License R-52. Section 6.7 c. 4.. an " observed inadequacy in the implementation of administrative or procedural controls." The inadequacy relates to the interval between Reactor Committee meetings.

According to Tech. Specs., Section 6.2 f., "the Committee shall meet at least quarterly."

Section 1.0 of Tech. Specs. defines quarterly to mean: " at 3-month interv ils [ interval not to exceed four months)..

..Any extension of these intervals shall he occasional and for a valid reason and shall not afTect the average as defined."

On the morning of March 2G.1993. prior to a Reactor Conunittee meeting previously scheduled for that day, the Director of the Reactor Laboratory checked the Tech. Spec.

requirements for meeting frequency and noticed that the four month limit mentioned above had been overstepped, the previous meeting having been held on November G.

1992.

The Reactor Supervisor was asked about the meeting schedule, and he replied that the meeting was scheduled in accordance with the Reactor Committee Charter (required in Tech. Specs..

.ction 6.2 d.) which stated that the required frequency was quarterly, not to exceed five months. The latest revision of the Charter, dated 1983, was thus found to be inconsistent with the current Tech. Specs.. which have been in effect since May 22.1990. Prior to that date, the previous Tech. Specs, did not define quarterly as not to exceed four months.

Evidently the definition of a quarterly interval introduced into Tech. Specs. in 1990 should have been incorporated in the Reactor Committee Charter, but was not. The Reactor Supervisor was aware of the requirements in the Committee Charter but was had been performed in a manner inconsistent with Tech. Specs. thoug noted that the average interval had not been extended because it has been the practice to schedule at least one meeting in each calendar quarter.

o The Reactor Supervisor was asked to check whether previous Reactor Committee meeting had been held in comp!!ance with Tech. Specs., or not. It was found that on three previous occasions. May 9 to September 23.1992, and May G to September 25.

1991, and May 9 to October 3.1990, the interval had exceeded four months. On these occasions the intervalincluded the Summer recess when many members of the Conuntttee are unavailable to meet.

The Reactor Committee was advised by the Reactor Laboratory Director on March 26.

1993, that a violation of Tech. Specs. had occurred and that this would be reported to the Commission in a letter report within 30 days.

J Remedial Actions The reason for the incorrect scheduling of the Reactor Committee meetings was the inconsistene" between the Committee Charter and the Tech. Specs.. In their meeting of March 2G.1993 the Conunittee approved a change in the wording of their Charter, so that the revised version now stipulates a quarterly interval, not to exceed four mon ths.

The responsibility for scheduling meetings was also clarified. It was agreed that the Reactor Supervisor should in future be responsible for ensuring that the Committee was properly informed of the required meeting dates, and that at each meeting of the Committee the scheduling of the next meeting shall be placed on the agenda.

All parties are now cognizant of the correct meeting interval requirements, and the actions taken should prevent a recurrence of this problem.
