NOTICE OF NONCONFORMANCE Omega Point Laboratories, Inc.

99901262/93-01 San Antonio, Texas Based on the results of an inspection conducted on March 2 through 5,
1993, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion VIII, " Identification and Control of Materials, Parts, and Components," of Appendix B to 10 CFR Part 50 requires, in part, that measures shall be established for the identification and control of materials, parts, and components, including partially fabricated assemblies, and that these measures shall assure that identification of the item is maintained throughout fabrication and use of the item.
Section 8.1 of " Comanche Peak Steam Electric Station (CPSES) Scope of Work for Testing Services," Revision 0, April 20, 1992, attached to Texas Utilities Electric Company (TU) Blanket Purchase Order No. B 0046494 dated April 24, 1992, requires Omega Point Laboratories, Inc., to have a documented quality assurance (QA) program that implements the applicable criteria of Appendix B to 10 CFR Part 50.
Section 4.1.11 of the TU " Test Plan, Ampacity Derating Tests of Articles Protected with the Thermo-Lag Fire Barrier System,"
Revision 3, March 3, 1993, and earlier revisions, requires TU and
" Associated Contractor Organizations" to provide all applicable quality control documentation for the fire barrier system mater-ials, cables, and installation for each test article.
Section 6.1 of the test plan requires that the cables used in the test program shall be traceable to the cable manufacturer, and shall be supplied by TU or Omega Point with documentation of traceability.
Section 1.3.2 of the Omega Point " Quality Assurance Manual,"
Revision B, September 4, 1990, states that the manual covers the full scope of Appendix B to 10 CFR Part 50.
Contrary to the above, the cable in a cable tray ampacity test specimen was purchased by Omega Point commercial grade from a distributor in 1988 and was not thereafter controlled as safety grade.
Traceability to the manufacturer was not established.
There was no documentation to show the manufacturer, date manufactured, model number, lot or reel number, or other quality attributes of the cable.
The ampacity test on the cable tray specimen was begun March 4, 1993.
